SCHEDULE OF EVENTS RFQ release date June 16, 2014 Bid closing date June 27, 2014 Contract effective date July 01, STANDARD TERMS AND CONDITIONS The State of Idaho Standard Contract Terms and Conditions are hereby incorporated by reference into this solicitation as if set forth herein in their entirety, and are located on-line at purchasing.idaho.gov/terms_and_conditions.html. The Terms and Conditions shall apply to this solicitation and the State of Idaho Standard Contract Terms and Conditions shall apply to any contract resulting from this solicitation. Failure by any submitting vendor to obtain a copy of such shall in no way constitute or be deemed a waiver by the State of the document, or any part of them. No liability will be assumed by the State of Idaho or Boise State University for a submitting vendor's failure to consider the Terms and Conditions in its response to the solicitation. 3. GENERAL INFORMATION 3.1 Definitions: University Boise State University Library Albertsons Library at Boise State University Binder Vendor awarded the Binder Contract Bindery Vendor performing the/location of binding services RFQ This Request for Quote and all addendums and attachments Standard Library Binding Institute Standard for Library Binding 3.2 Contract Overview The Albertsons Library is the main campus library serving the Boise State University community. The current contract for commercial binding requirements expires on June 30, Boise State University is soliciting competitive bids to establish a new binding contract as specified within this document. This contract shall be for a period of one (1) year with options for two (2) successive one (1) year extensions. Binding services shall be made in accordance with the specifications identified in Section 4 of this document. 3.3 Correspondence and Addenda All correspondence will be in writing. In the event that it becomes necessary to revise any part of this RFQ, addenda will be posted at vpfa.boisestate.edu/purchasing/. The Bidder is responsible for monitoring the BSU Purchasing web site for any updates or amendments. Any oral interpretations or clarifications of this RFQ shall not be relied upon. All changes to this RFQ must be in writing and posted at the BSU Purchasing web site to be valid. 3

4 3.4 Invoicing The Bindery shall provide two (2) copies of an itemized billing to the Library for each shipment within seven (7) calendar days of delivery of the shipment to the Library Each invoice shall list each type of binding treatment separately and include the number of items so treated, the charge per item, the date the items were sent to the Bindery and the total charge for each invoice. 3.5 Contract Term Contract shall commence July 1, 2014 and end June 30, This agreement may be extended for an additional twelve (12) month period at no change in contract amount or conditions if agreed to in writing by the binder and the university prior to its expiration. Two (2) such successive extensions may be authorized prior to re-bidding. 3.6 Termination Failure to complete any terms or meet other provisions of this contract shall be cause for termination of this Contract. Failure to satisfactorily provide binding services of an acceptable quality and in a timely manner will be cause to terminate this Contract at the discretion of Boise State University. Boise State may terminate this Contract without cause with 30 days written notification. The Contract may be canceled immediately upon written notice by Boise State University to the Binder of any binding error not up to standard. 3.7 Award Basis All bids will be evaluated based on the criteria listed in Section 6 and their corresponding point value. Award will be based on the highest point value awarded. 1. The bid will be awarded on the following factors: Bid prices (usage as noted) 500 points maximum Samples 500 points maximum Total available points 1,000 points maximum The maximum number of points will be awarded to the low dollar bid. The low bid will be divided by the next low bid with the result being multiplied by 500, the maximum number of points, which equals the number of points. Example: Low Bid $45.00, Next low bid $60.00 $45.00 divided by $60.00 = 75 multiplied by 500 = 375 points Samples from each vendor will be rated by Library staff based on the quality of samples received. See Section 6.7 for a list of the required samples. The vendor with the highest sample rating will receive 500 points and all others prorated against the high score. If any vendors tie for high sample rating, each vendor will receive 500 points. Information requested in section 6.5 and references are rated on a pass/fail basis. 4

5 3.8 Bid Prices Prices quoted shall include all pickup, transportation and delivery charges Bid prices shall include ABLE software or a comparable computerized binding system and supplies required for the Library to prepare materials for binding Additional charges for handwork at an hourly rate for extensive repair, and cloth and board pockets shall be specified on the Schedule of Prices/Surcharges (Attachment A) Charges for paper pockets, excess lettering/stamping, call numbers, excess height or thickness, insertion of security strips, the Boise State University imprint, and stubbing etc., shall be included in prices quoted for each binding category Price escalation clause: a. The successful vendor may request in writing upon Contract renewal a price adjustment, if applicable. The price adjustment will be effective the date of the Contract renewal. Bid prices shall remain firm the first year of the Contract. b. The price adjustment will be based on the percentage of the U.S. Department of Labor Consumer Price Index (CPI-U), All Items Category. The Base Month for the price adjustments will be the month three months prior to the month of the Contract award. c. The price adjustment will reflect the unadjusted percent change in the All Items Category for the twelve (12) month period immediately after the base month. d. The CPI base index for price adjustments will be given at time of award. 3.9 Compliance and Specifications The Binder shall be certified by the Library Binding Institute and maintain that status for the life of the Contract. All binding shall adhere to the most recent edition of the Library Binding Institute Standard for Library Binding (herein referred to as the Standard), whether such binding be periodicals, monographs, theses, or other materials The timeframe for conversion to a new system is one (1) month from Contract award The Library reserves the right to specify binding styles and treatment for any and all items The Bindery shall provide specifications for miscellaneous/special binding services/products available that are quoted in this bid but have not been described herein, such as economy paperback binding, pamphlet binding, archival boxes, etc Prior to Contract award and at any time during the Contract period, the Bindery shall permit representatives from the Library to inspect the Bindery during its normal working hours Bids received without the required samples and documentation will be considered non-responsive Any changes in traditional binding methods or materials used by the Bindery must be approved by the Library Written performance reviews may be provided by the Library indicating the Library's degree of satisfaction with the performance of the Bindery. Such reviews if needed will address: a. General Contract compliance b. Quality of work c. Delivery schedule d. Error rate Upon receipt of a written negative performance review, the Bindery shall have fourteen (14) days to rectify all errors and demonstrate their ability to comply with the Contract Should the Bindery repeatedly fail to perform and supply as hereto set forth in section 3.9.8, Boise State University may cancel this Contract with cause by giving ten (10) days written notice. Cancellation for cause shall be at the discretion of Boise State University and shall be, but is not limited to, failure to 5

6 supply the materials, equipment, or service specified within the time allowed or within the terms, conditions, or provisions of this Contract The successful vendor may not cancel this Contract without prior written consent of the Boise State University Purchasing Department Boise State University may cancel this Contract without cause upon thirty (30) days written notice Subcontracting All binding shall be done on the premises of the Bindery unless permission to do otherwise is granted by the Library Subcontracting is allowable for pickup, delivery, and transportation of the volumes between the Library and the Bindery No subcontracting other than transportation will be permitted without the written approval of the Boise State University Purchasing Department Insurance and Indemnification The Bindery shall insure, at no additional cost to the Library, all materials against loss or damage from any cause, from the time they leave the Library until they are returned. The exception shall be rare or expensive volumes (over $350.00/ea) for which the Library will make the following special arrangements in advance. a. If the Bindery cannot pick up rare or expensive volumes, then the Bindery will supply preprinted mailing labels either prepaid or with a mailing account number for use by the Library in mailing and insuring the material(s). b. Regardless of whether rare or expensive volumes are mailed or picked up, the Library will alert the Bindery prior to arrival of shipment or pick up details Liability for items lost, damaged, or destroyed will be the replacement cost (including processing and binding) as determined by the Library The successful Contractor and subcontractors, if any, shall indemnify and hold harmless and defend Boise State University and the State of Idaho, their officers, agents and employees from and against all claims, damages, losses and expenses for bodily injury, sickness disease or death, or to injury to or destruction of tangible property including loss of use resulting there from, including but not limited to attorney s fees, arising out of or caused in whole or in part by the negligent acts or omissions of the successful Contractor, a subcontractor, anyone directly or indirectly employed by them or anyone for whose acts they may be liable, but only to the extent caused in whole or in part by negligent acts or omissions of Indemnitor. The insurance company providing the general liability insurance certificate must have an insurance company rating of A or higher In addition, the successful Contractor and subcontractors, if any, must, at their sole cost and expense, procure and maintain through the term of this Contract the types and limits of insurance listed in Attachment C Insurance Requirements Within ten (10) days from notification of bid award, the successful Contractor and subcontractors, if any, shall deliver or fax to the University certificates of insurance with respect to ALL such insurance including worker s compensation in a form reasonably satisfactory to the University. The general and automobile liability insurance certificate must name Boise State University and the State of Idaho as additional insureds and must contain a written provision that, should any of the above-described insurance policies be canceled or non-renewed before the expiration date thereof, the issuing company must notify the University in writing, by certified or registered mail, receipt requested, at least thirty (30) days prior to any cancellation or non-renewal of any such insurance. 6

7 3.12 Communications The Bindery shall be willing to accept collect telephone calls or provide a toll-free number, electronic mail or fax transmissions A representative from the successful Bindery shall visit the Library at the time the Contract is awarded to demonstrate special binding procedures and train library personnel to use the binding software. The representative shall visit the Library periodically, as required by the Library, and be available on request. The representative shall be thoroughly familiar with the terms of this Contract; shall have in-depth knowledge of the technical aspects of library binding and the operations of the Bindery s/he represents; and the preservation of library materials Packing, Pickup, Delivery The Bindery shall supply preprinted address labels and mailing cartons suitable for packaging of volumes for shipment to and from the Bindery. The cartons shall have at least a 250-pound bursting strength The Library shall pack all volumes to the Bindery: a. Periodicals will be secured together by binding unit. b. Sets that require identical bindings will be boxed separately and secured together by binding unit. c. Volumes requiring special handling will be designated and boxed separately The Bindery or the Bindery's subcontractor shall make regularly scheduled pickups and deliveries; materials shall be bound and returned within twenty-eight (28) calendar days from pickup date except as noted in section The Bindery or the Bindery's subcontractor shall provide pickup and delivery to the Technical Services area of the Library located on the second floor of the library building Shipment integrity shall be maintained. All items picked up in one shipment shall be returned together. If exceptions must be made, the Bindery must notify the Library in writing, , or fax. Consistent receipt of incomplete shipments may be cause for cancellation of this Contract Standard delivery: Materials shall be bound and returned within twenty-eight (28) calendar days from the date of pickup except when the Library and the Bindery agree upon a different schedule for specific items or shipments. Consistent or excessive delays may be cause for cancellation of the Contract Errors and Delays Bindery errors in lettering, binding or collation shall be corrected by the Bindery at no additional charge to the Library and shall be corrected and returned to the Library within fourteen (14) calendar days of the Bindery's having received the item for correction. Any additional transportation costs resulting from Bindery errors shall be paid by the Bindery. The Bindery shall replace materials that cannot be repaired within sixty (60) calendar days. If replacements cannot be obtained, the Bindery will be billed at a rate equal to the current subscription price per issue plus $50.00 staff time. Errors, which require the skills of the Library staff to correct, shall be subject to Section 3.9 (Insurance and Indemnification) Any item not returned to the Library within twenty-eight (28) calendar days will be considered lost, and the Bindery will be billed under the provisions of Section 3.9 (Insurance and Indemnification) Uniform Binding of Periodicals and Sets The Bindery shall specify in detail the method and time frame of conversion of data for uniform binding of periodicals from the information provided by the Library. The conversion of data and ongoing records maintenance shall be carried out at no additional cost to the Library The Bindery shall make every effort to match the binding pattern, placement of lettering on the spine, color of stamping foil and cloth of existing bound volumes of periodicals. 7

8 The Bindery shall provide the Library the computerized means necessary to achieve uniformity for binding periodicals and sets. This information shall consist of: (a) the entry and/or title, worded precisely as it will appear on the spine of the volume; (b) a profile, in correct sequence, of the variable information that must be stamped on the spine; (c) the correct color of cloth and stamping foil Computer Service The Bindery shall provide a computerized software program that will maintain Library binding records allowing for additions, deletions and corrections to the records The system shall be able to produce two-part binding slips for each title, one of which can be retained and used by the Library Each printed binding slip shall include: a. The fixed title and subtitle or other entry, worded precisely as it will be stamped on the spine of the volume. b. A profile, in correct sequence, of variable information that must be stamped on the spine. c. An indication of the correct color of cloth and stamping foil. d. An area to indicate instructions for custom collation as specified in the latest edition of the Standard. e. The system shall be able to accommodate the processing of monographic items submitted for binding or rebinding as a single unit The system shall allow the creation of hard-copy lists in alphabetical order of those titles prepared for binding on a shipment-date basis The Bindery shall be responsible for creating a database from the Library s electronic and/or paperbinding records, installing the program, providing training sessions to the Library staff, and providing system manuals and consultations. The Library will transmit the data in machine-readable form to the appropriate Bindery according to a mutually agreeable schedule. Local binding data shall be the property of the Library The Bindery shall supply a software program to operate the system and produce binding slips. The software program shall be compatible with existing Library computing facilities necessary to transmit information or the Bindery shall supply the necessary computer hardware. The Library currently uses ABLE software Inspection All volumes bound for the Library shall be carefully inspected by the Bindery for defects in construction, collation or lettering and shall be wiped clean if necessary before packing and shipment to the Library All shipments shall be guaranteed to be free of insects or vermin. 4. SPECIFICATION FOR MATERIALS 4.1 The Standard for Materials All materials used in binding items included in this Contract shall meet or exceed the Standard. These standards shall apply as written for papers, bindery's board, cloths, adhesives, thread, sewing tapes, stamping foil, and covering materials. 8

9 5. SPECIFICATIONS FOR STANDARD BINDING OF MONOGRAPHS AND PERIODICALS The Standard for Binding All techniques used in binding items included in this Contract shall meet or exceed the Standard. These standards shall apply as written for papers, bindery's board, cloths, adhesives, thread, sewing tapes, stamping foil, and covering materials. 5.1 Definitions Monographs: For the purpose of this Contract, a monograph is defined as one (1) piece of graphic material submitted for binding or rebinding as a single unit without reference to another unit, or with no demand placed on the Bindery to match the unit to another. Although some items in this category may be bibliographically classified as periodicals or may be part of a set, the Library assumes responsibility for uniformity of cloth color and cover sampling Periodicals: For the purpose of this Contract, a periodical publication is defined as a single piece of graphic material bound separately, or a series of two (2) or more serially numbered units bound together, for which the cloth color must be selected, and the cover stamped with information so as to match other publications in the same set or series Theses, projects or dissertations: For the purposes of this Contract, a thesis or project is defined as one (1) piece of graphic material submitted for binding as a single unit. All theses and projects shall be bound using the blue buckram color designated by the Library. The foil used for lettering shall be gold. Spine stamping will be according to the following format: title, surname, year and imprint. The title may be truncated at the complete word. In the event of truncation, the symbol... must follow the title. The Library shall provide a completed binding slip for each thesis and project and shall assume responsibility for collation, and providing the Bindery with the correct spine information. The Bindery will assume responsibility for the uniformity of color and cover stamping. 5.3 Examination, Inspection and Collation The Bindery shall examine all volumes to detect damaged leaves and peculiarities of paper or construction that might make first time binding or re-binding inadvisable The Bindery shall use standard collation as specified in the latest edition of the Standard unless otherwise advised. Incomplete or imperfect volumes shall be returned unbound unless the Library has provided instructions to bind as is The Bindery shall be able to provide custom collation as specified in the latest edition of the Standard Special preparation a. In general, the Library will provide instructions for folded plates, plates that bleed across the margin and pockets and the Bindery shall follow the latest edition of the Standard. The Bindery according to the provisions of the Standard will handle items for which the Library has not provided special instructions. b. The Bindery may proceed with up to one hour's handwork at the contracted hourly charge for any item needing extensive repairs. c. Upon encountering a volume requiring extensive stripping, folded plates or other handwork totaling more than one hour's time, and when no instructions have been noted by the Library, the Bindery shall contact the Library for further instruction. d. In no case shall a Bindery proceed with binding a volume having folded plates, or text or plates bleeding across the margin that would require the loss of any printed or illustrative material, unless the Library has given explicit written permission Mending 9

10 a. All adhesives, papers, and other materials used for mending must conform to the latest edition of the Standard. 5.4 Removing Backs Rounded and backed text blocks which must be rebound, and for which it is not possible to preserve the original sewing structure, shall have boards removed and the old rounding and backing taken out by nipping, before the edge is trimmed or milled For text blocks which must have the spine edge trimmed or milled away in preparation for oversewing or double-fan adhesive binding, as little as possible (and not more than 1/8 inch) of the edge shall be removed, in order to preserve as much of the inner margin as possible Periodical issues which are saddle-stitched and which cannot be sewn through the fold (e.g., when they must be bound together with non saddle-stitched issues) shall be prepared for adhesive or oversewing by slitting through the fold rather than by trimming or milling All staples must be pulled from side-stitched text blocks to provide an inner margin of maximum width. No text blocks shall have staples removed by trimming or milling. Staples shall be removed prior to their being sewn through the fold. 5.5 Construction Methods Technical specifications for leaf attachment, endpaper construction, trimming, gluing, rounding and backing, case-making, and casing-in shall follow the latest edition the Standard If an item cannot be bound according to the Standard, the following specifications, or to specific instructions, the Bindery shall either contact the Library for instructions or the item shall be returned by the Bindery with an explanation. In addition to the Standard, the following shall apply: a. Leaf Attachment 1. In general, the Library will specify the instructions: "recase", "oversewn", and "sew through the fold". All other bindings shall be presumed to be adhesive bind. (In case other methods are available, the Library may designate other methods). 2. Rounding and backing shall be done for previously bound, rounded and backed textblocks or volumes with excess swelling of the spine from sewing. For all other serials or monographs that are double-fan adhesive bound, sewn through the fold, oversewn, or flatbacked binding is acceptable. 3. The Bindery may select a different method from the Library's instructions for recase and adhesive bind when the Library's instructions are not possible or when a different binding method would provide a stronger and/or more flexible binding. The Bindery may select a different method from the Library's instruction to sew through the fold when this method is not possible and when another method is possible under the Standard. 4. The Bindery may use sew through the fold, adhesive or recase bindings even when not indicated by the Library's instruction if, in the Bindery's judgment, the volume would be better bound by one of these treatments. 5. The Bindery shall not use the oversew method on volumes which are brittle or have narrow margins (less the 5/8") except by permission of the Library. Exceptions may be made for periodical volumes with clay-coated paper stock and/or thick covers. In these cases, the allowable margin is 3/8". 6. Music scores shall always be bound by the method that allows the most flexible opening possible. This will most often be by sew through the fold. 7. For volumes that the Library has designated for recase, the Bindery shall make every effort to honor that request in order to maintain the integrity of the text block. For these volumes, 10

11 alternate end-sheet constructions (tipped-on or whip-stitched) are acceptable as appropriate for the particular volume. b. Lettering 1. Lettering on the spine must conform to the requirements set forth in the latest edition of the Standard. 2. Characters must be available in both upper and lower cases. In addition, standard punctuation marks shall be available in sizes matching letters and numbers. 3. Lettering size shall be in proportion to volume thickness. Call numbers shall be as large as spine width can accommodate. 4. The Library requires black, white and gold stamping foils. 5. Stamping shall be done in horizontal lines on the spine. In cases with narrow spine widths, lettering may be vertical, beginning at the top and running down the spine. 6. For periodicals, the Library will generally include title, chronological and/or date information and the Boise State University imprint. The Library requires that the call number for all periodicals be included on the spine. Lettering for periodicals shall match previously bound volumes in color, placement and content. 7. For monographs, the Library will generally include title, author surname, and Boise State University imprint. There shall be a "break" between the author and title. The Library requires that the call number be included on the spine if possible. c. Stubbing 1. When periodical parts of different heights are to be bound together, stubbing should be used whenever practical to make up for size differences. In no case shall one piece be trimmed excessively for the purpose of making it conform to a smaller piece with which it must be bound. 2. Stubbing shall be added to volumes to compensate for thick pockets on back covers, and to correct text blocks which flair out towards the bottom, not at the top. d. Flush Bottom 1. When periodical parts of different heights are to be bound together, the resulting text block should be flush at the bottom and not at the top. e. Pockets for Supplementary Materials 1. Pockets shall be made of alkaline paper, tear-resistant fabric, or fabric and board, depending on the weight of the materials they are designed to protect. They shall be constructed so that the materials they contain are firmly supported, and are not easily damaged as they are inserted or removed from the pocket. f. Miscellaneous Binding Treatment 1. Different styles of binding other than those specified in this Contract may occasionally be requested by the Library. The Bindery shall supply specifications for other available binding services, and rates charged for those services in Attachment A on the Schedule of Prices/Surcharges. g. Theses and Dissertations 1. Effective with Class of 2014 and beginning with Spring 2014 Commencement, Boise State University will submit electronic orders for theses and dissertations to be printed and bound by the bindery. The Library and Graduate College will use an Excel file template, will include the same document metadata that is typically included on a standard binding slip, and will include a URL to the electronic document on the Albertsons Library Institutional Repository site ScholarWorks. This Excel file will be sent to the bindery as an attachment. The bindery will print the specified number of copies, bind them, and deliver them to the Library with other bindery shipments. The Library will be billed separately for the printed and bound electronic theses and dissertations. 11

12 6. RFQ RESPONSE OUTLINE Note: To aid in the evaluation process, all quotations are REQUIRED to include complete responses to Section 6 and Attachment A. Failure to comply may result in your proposal being found nonresponsive. 6.1 Bidders shall submit the completed Schedule of Prices/Surcharges sheet (Attachment A). 6.2 Submit the completed Signature Page (Attachment B), signed by a company official who has full authority to enter into this Contract. The Signature Page shall be attached to the front of the bid response. 6.3 Submit samples of ALL binding types requested in the Contract for evaluation by the Library including double-fan adhesive bind, oversew, recase, sew through the fold, economy paperback and theses, projects and dissertations. Samples will be returned to the bidders at the Library s expense. Include samples that demonstrate computerized binding system and products. 6.4 Subcontractors: If subcontractors are included in the completion of this bid, please provide the following information for each subcontractor: company name, address, phone number, and contact person. 6.5 Service Requirements: Contractors MUST provide the following information: Include a statement giving the number of years the Bindery has been open under current ownership or the number of years under the current management Include a statement of the approximate gross sales completed in the last two (2) full years of operation Provide current proof of certification from the Library Binding Institute (LBI) Certify that you will maintain certification from the LBI for the life of this Contract Certify that you will comply with a one-month time frame for conversion to a new system Certify that you will make a concerted effort to return together, all items picked up in one shipment or notify Library as soon as possible Certify that you will provide a computerized software program that will maintain Library binding records Provide a detailed description of automated systems currently operational and a detailed breakdown of any associated charges Describe plant resources, including number of full-time employees, part-time employees, number and types of binding machines etc. 6.6 References - Submit a Reference List with the information listed below: Three (3) academic accounts with comparable or larger binding budgets will be provided as references for the Contractor. The three (3) references must be current references for which continuous services have been provided during the last three (3) years. For each reference supply the company name, address, phone number, address and contact person. Describe job duties, state length of employment for each reference, and state annual binding dollars spent with your Bindery Three (3) accounts will be provided as references for the subcontractor, if any. All references must be current for which continuous services have been provided for two (2) years. For each reference supply the company name, address, phone number, and contact person.
