2 * * * INFORMATION * * * Artist Eligibility Any federally enrolled citizen of one or more of the Five Civilized s (Cherokee, Chickasaw, Choctaw, Muscogee (Creek) or Seminole), who is 18 years of age or older, may enter. ENTRANTS ARE REQUIRED TO ENCLOSE a copy of CDIB or Tribal Registration Card. Art Sales All entries must be for sale. The Five Civilized s Museum retains a 30% commission on each entry that is sold. At the conclusion of the show, checks will be written to any artist whose work has sold and will be mailed by Nov. 15 th to the address provided on the entry form. Entry Specifications Artists are limited to three (3) entries total. Entries must not be more than two (2) years old. **A typed interpretation or legend must accompany the work.** Entry Identification The enclosed entry identification labels should be completed and attached to the back of each entry or on the bottom of each sculpture. Also, be sure to complete the labels located on the Entry Form. Guidelines Entries must depict a Five Civilized s subject pertaining to a known tribal folklore story, legend or myth. Award Information First Place - $ Second Place - $75.00 Third Place - $50.00 People s Choice - $ There will also be two Honorable Mention ribbons awarded. Special Prize The People s Choice Award will be announced at the closing reception. The winning artist will be notified and presented with a ribbon and their prize monies. Their name and the title and image of their winning piece will be posted on our website, Facebook and Instagram. Reception A reception for artists, their families, museum members, and special guests will be held Oct 27, 2017, from 5:00-7:00 p.m. in the upstairs art gallery at the museum. Participating artists and their families are considered our guests and will be admitted free. Artists may pick up their entries at this time.

3 Return Shipping for Entries Enclose a check payable to the Five Civilized s Museum for the amount of return shipping. IT IS YOUR RESPONSIBILITY TO CHOOSE A CARRIER AND SPECIFY THE INSURANCE AMOUNT FOR RETURN SHIPMENT IF IT IS TO BE RETURNED BY MAIL OR OTHER CARRIER. Pick-Up of Entries Following the show, the artist must pick up his/her art in person, unless authorization from the artist for the art to be picked up by someone else has been supplied in writing and given to the museum in advance. Publicity The museum has the right to photograph and/or reproduce any or all works in the show only for publicity and for permanent file purposes. Participation in this show constitutes compliance with these terms. Liability All possible care will be taken, but the museum will not be responsible for any damage, loss, or deterioration of any object while in transit or while on display in the museum. Artists assume all risks relating to artwork entered.
