Is it any wonder Ephron s book went straight to the best-seller list upon its release last year? Couldn t most every one of us say, We look good, except for (fill in the blank)? While the nervous Nellies among us (like me) would rather learn to live with our sagging jowls than risk going under the knife, it doesn t take a rocket scientist to know that such conservatism is fast losing ground. Thanks partly to the aging of the baby boomers and thanks partly to the Dr s of the world, the popularity of cosmetic surgery may soon outpace that of the potato chip and that s saying a lot. In 2005, more than 2 million Americans elected to undergo cosmetic surgery, according to the American Society for Aesthetic Plastic Surgery, representing a whopping 119 percent increase since It also is becoming more mainstream. It isn t just the rich and famous [who are getting cosmetic surgery] it s everyone, notes Thomas Whetzel, M.D., a plastic surgeon and professor of surgery at UC Davis Medical Center. Whetzel believes the demand for cosmetic surgery will continue to grow because today s consumers are less willing to accept what is. If they have a little extra skin from childbirth that s hanging from their abdomen, they ll say, Oh, I need to have that cut off, he says. The previous generation probably wouldn t have even thought about it. For those who are thinking about it, here s a look at 10 hot topics from procedures to sociological trends straight from the lips of local plastic surgeons 6. The demographics are shifting. Although women are still winning the cosmetic surgery sweepstakes by a landslide, accounting for some 91 percent of those who underwent cosmetic procedures (surgical and nonsurgical) in 2005, a growing number of men are slowly trickling into the race. It s still a low percentage, says Toft, but I am seeing more men. Most are motivated by a desire to compete in the workplace, notes Toft. They re often competing with younger guys in the business world and want to look less tired, more refreshed. The demographics are shifting in other ways, too: Glover notes he s seeing an increasing number of African-Americans and Hispanics ethnic groups you didn t see as frequently in the past. As for couples scheduling surgery together a trend cited in a recent survey by the American Academy of Facial Plastic and Reconstructive Surgery Sacramento couples aren t exactly breaking any records, according to local surgeons. 7. Help for aging necks. In I Feel Bad About My Neck, Ephron laments that a neck lift can t be performed without a face lift that it s all one big ball of wax. But according to Toft, a neck lift can be performed by itself, though the more common scenario is to combine the procedure with a face lift. Very frequently, people s necks go faster (age faster) than the rest of the face, he says. During a neck lift, the sagging neck muscle (platysma) is tightened and excess skin and fat is removed, resulting in a taut, more youthful neckline. In minor cases, Toft says, a little liposuction under the chin will do the trick. But those whose necks have deteriorated into a waddle will probably need a full face lift (which includes a neck lift) for best results.

Isn t that the whole thing when it comes to New Year s resolution? I wanna look better! Dr. Pete Hilger: Sure. Host: I wanna get a facelift, reconstructive surgery! What do I need to know about it? PH: Well, you want to make sure the physician that you re seeing is capable and interested in you, and there are some benchmarks that you can use to evaluate that. We think that you should choose a Board-certified surgeon; you should choose somebody who specializes in facial plastic surgery, and your surgeon should be a member of an ongoing educational organization, such as the American Academy of Facial Plastic and Reconstructive Surgery. So do your homework on the front end. Make sure the physician that you see matches up with you and shows you before-and-after photographs of what you re looking to do. And we have a website that can help you identify a good surgeon at facemd.org. It also has a lot of great background educational information for patients that can help you put all of this in context. Host: And that s all linked up to our website as well, folks. So what is some of the latest technology in reconstructive surgery? PH: Well, there s a lot of new injectable fillers out you ve heard a lot about Botox in the last year but even now there s some newer fillers out on the market, and in your area more than half of our members responded that injectable fillers are the most common procedure that s done, and this has been increasing over the past year or so. The minimally invasive-type treatments that give you no downtime are the newest things out on the marketplace. There are some other kinds of resurfacing techniques that can freshen up your skin that have no downtime as well. Those are the newer things out; there are a lot of traditional surgeries that are still available that can really make a nice difference for you; but the goal is to have a nice natural-looking outcome you don t want to look like you ve had surgery. Host: Yeah, I ve seen these women who look like they ve gone through a wind tunnel PH: Yes. Host: And it just looks ridiculous, it really does. Now, Botox. How long does an injection or you re talking about this new form of it how long does it last? PH: Oh, several months at least. And we find that as people often have it done over several times, the effects even seem to last longer. So a lot of people we may see two or three times a year and you re doing little things to keep them

4 looking youthful and natural. Absolutely that look of being stuck in a wind tunnel is very unnatural, very unattractive, and we certainly try and avoid that. My patients come in and that s one of their biggest concerns, is Don t make me look like Melanie Griffith. Host: Yeah, Joan Rivers (both laughing). And do we have more advancements coming up soon that you see that you can give us a little tip on? What we might be looking out for? PH: Yes, I expect that even in the next year or so we re going to see some of the newer fillers; there are some really nice minimally-invasive things that can be done with a laser-type device to help you look more youthful. And even some of the traditional procedures like facelifts have been modified to give patients less downtime but still some pretty darn gratifying results. Host: Ok What do most of your patients come in for? PH: The most common things are injectable fillers. Rhinoplasty is a very common procedure, it s the most common facial. Host: I m sorry, Doc, but why in the world- if you have a nose job, why do they call it rhinoplasty? It just sounds awful. PH: (laughing) It comes from old Greek and Latin terms. Host: Yeah, but why do you have to be compared to a rhinoceros when you walk in there? I don t get it. PH: Rhino means nose. And plasty means to reshape. So rhinoplasty is reshaping your nose. Female Host: And rhinoceros means nose dinosaur. Host: Ok, nose dinosaur, there you go. Good, I m glad we clarified that (laughing). Ok, so most of your people are coming in for nose jobs and injections, and I m sure breast augmentations are popular ones. PH: Well, not in our field. We specialize in just facial aesthetic surgery, and I think that s really an important concept for patients to appreciate. Make sure the physician you re seeing specializes in the area that you have needs so I wouldn t go to a foot doctor if I wanted to have my face lifted, and I would suggest that you see a facial aesthetic surgeon a facial plastic surgeon if you have an interest in improving your facial appearance. Host: And what hospital are you associated with? PH: I am affiliated with the University of Minnesota in Minneapolis, St. Paul, and teach there. Host: You must have been pretty embarrassed last night at what you guys showed up with American Idol people, huh? Minneapolis didn t look good. PH: Well, I wasn t there; I was off taking care of the players at the Wild game. Host: Oh, ok. (Mumbles joke about a plastic surgeon in a face-off). I m right on today with all these jokes. That s funny stuff. That s good stuff. I m sure you must know all the plastic surgeon jokes, right? PH: I certainly don t; you probably have a bunch that I don t know about. Host: Ok, now what are some of the dangers of Botox? You re talking about all these injections that help out, but there s got to be some downside to this stuff. PH: Actually, Botox is an amazingly safe treatment option, and you know, what you re doing is weakening, selectively, some muscles to give you a more flattering appearance. The biggest challenge I see is sometimes we under-treat people who need a little bit more in a week or so. Host: What do the Botox treatments run? What do they cost? PH: Well, it depends on the areas that you re being treated, but it s several hundred dollars for an initial treatment session. Female Host: How often do you have to go?

5 PH: Uh, some people see us every four to five months; some find it lasts longer; there are some folks that see me every six or eight months. Host: Ok, and what about collagen for the lips? PH: Yeah, injectable collagen s been around for a couple of decades; there are some new fillers out like Restalyne, Juverderm and Radiesse. And these are fillers that are much more long-lasting; they may last anywhere from six months to a year. Host: Now Doc, have you done any of this yourself? PH: You mean on myself? Host: On yourself, yes. PH: Oh, sure. Well, I have my fellow, someone I m teaching- Host: I don t mean you do it yourself, inject yourself; I m saying, do you have plastic surgery? PH: Sure. Host: On what? PH: For me? I ve broken my nose a bunch of times and had to have it fixed, so it looks better and I can now breathe through it again. I ve had Botox injections from my fellow, so that my forehead s a little more relaxed than it used to be. Female Host: How come you keep breaking your nose? PH: I grew up in a big family (laughing). Host: At what age do you usually find people are starting to go into it? PH: We find that some of the less invasive treatment options, people are coming in even in their thirties. So of the issues like rhinoplasty if you ve got a bump on your nose you know about that by the time you re sixteen, eighteen years old so those kinds of procedures are often done in high school or college age. And some of the facial rejuvenation procedures like some of the less invasive facial type procedures, people are coming in their late thirties, forties, fifties. Host: I m just looking at you picture you ve had this done and you look like hell, Doc. (Laughing). No, I m kidding. That s like me going out and trying to sell Rogaine doesn t work! Look at me, I m a Rogaine salesman No, I m just kidding. I m sure you look very young. And does it make you feel a lot better? PH: Absolutely, and that s what this surgery is about: not trying to look like somebody else, but trying to make you feel good about yourself and to look very natural. Host: What s the problem with just dealing with the cards you re dealt, just playing that hand? Female Host: Feeling good on the inside? PH: Yeah, that s exactly- you know, our surgery is trying to reconcile how you look on the outside with how you feel on the inside. Nobody wants to look old and crabby and irritable, so you want to have a natural, relaxed look. You still want to look like yourself, though. Host: You know, I have another solution: we ve covered all the mirrors in our house, and I ve just gone with that. Female Host: Good idea, Host: What do I care? I feel fine. I don t care what I look like to you guys.
