1 MUSIC NAMA 2016 ARTIST ENTRY FORM AMIBIAN ANNUAL Please ensure that you submit the following at your nearest MTC/NBC office along with your entry form this is compulsory to qualify!!! 1x image disc which contains the band/individual image and the image of the album cover. (NOTE: images should be high resolution and sized between 500 to 1000 pixels high and wide not exceeding 1 MB). 2x original copies of the album with its cover ( entries without this will be disqualified unless entering for the Best Single category);; 1x copy of the single entry in MP3 format clearly marked (only for non album entries);; Entries for Best Music Video are to provide 1x original copy;; 1x MP4 formatted copy (clearly marked);; and a Namtunes/Youtube link for the video entered;; Should you enter more than one album or single, kindly make use of a new form for every entry. Multiple entries on the same entry form will automatically be disqualified;; Copy of ID/Passport Ensure all copied CDs/envelopes are clearly marked.
