STUDENT HEALTH CENTER PROJECT #TM DIGITAL X-RAY UPGRADE WITH PACS IMAGE DATA SYSTEM AND RADIOLOGY LAB RETROFIT SCOPE OF WORK The primary focus of this project is to replace the Student Health Center s obsolete Film X-ray machine with a new Digital Radiography (DR) X-ray machine and electronic imaging/data storage system, as well as to perform a necessary retrofit of the existing radiology rooms to accommodate the new equipment. This project is critical to assuring continued quality health care to our students. The scope of this project includes the removal of the Center s antiquated and obsolete Film X-ray machine & film developer, housed in HC 112B and C, which has become increasingly unreliable and is no longer supported or repairable. In addition, this project will require moderate construction in order to retrofit the existing space to accommodate new equipment and upgrade the X-ray rooms to conform to current health and safety standards. The overarching objective of this scope of work is to commission a single vendor to deliver a comprehensive turnkey digital x-ray laboratory complete with all the necessary equipment, electronic imaging and data storage capability, utility infrastructure, telecommunications and physical location improvements necessary to provide a fully operational digital x-ray laboratory that is HIPAA compliant. To this end, please review the attached three part proposal that provides an in depth and detailed look at the requirements for each element of this important project. A. The system combines many advantages of a digital radiography unit: excellent image quality high efficiency through fast time to image good ergonomics reduced radiation dose and flexibility for integrating into other digital infrastructure The Essenta DR system supports both connection to a Radiology Information System (RIS) and to DICOM-compatible workstations, archives and printers. Applications: Typical applications for Essenta DR are to be found in general radiology departments. The full flexibility of procedures is achieved by automating the system and dispensing with cassettes. The user has the following exposure techniques available: Detector exposures with a vertical, horizontal and/or oblique radiation beam Free Exposure technique for CR systems of Philips or other vendors and film cassettes. 2

CEUs are not available in all cases. Please read Guidelines for more information, which will be provided to you during the scheduling process. Education Hours: Mon Fri 8:00am to 5:00pm, except Monday and Friday are half-days to allow for trainer s travel. Note: Philips personnel are not responsible for actual patient contact or operation of equipment during education sessions except to demonstrate proper equipment operation. The Eleva workspot supports DICOM Greyscale Display Standard. Calibration of Eleva workspot and the receiving DICOM node will result in consistently same high image quality. DICOM Image Export also includes the DICOM Storage Commitment service, allowing the Eleva workspot to be informed by storage destination if images have been securely stored. 12 B: Construction/Retrofit of Student Health Center X-ray Laboratory: Work closely with or under direct supervision of primary x-ray equipment vendor. Follow all CSULA university guidelines & procedures forbuilding safety and supervision of employees. Work closely with our University Facilities Planning & Construction Office to meet all construction requirements and applicable construction codes. New DR System should be converted to appropriate size circuit breaker. Work closely with campus Environmental Health & Safety and provide necessary third party Physicist authorized to certify safety standards of finished construction.. Vendor must meet our university procurement requirements for General LiabilityInsurance and workers compensation requirements. Any design & redesign work required must be reviewed and approved by appropriate University authorities. DESIGN: All architectural and engineering work necessary to complete the project described above, including: Preliminary/schematic design and design development work as required to obtain sign off by the Clinic Weekly Customer meetings are included. All required survey and testing work. Construction document production (drawings & specs). Include (1) physicist report for required lead shielding include the post installation physicist testing certification. Copies of the construction documents as required by all parties and other miscellaneous printing costs including read-only CADD files. All review and approval work and fees as required by local agencies or other governmental authorities having jurisdiction over the work. Any redesign work required by review and approval authorities. Any pre-construction meetings. Shop drawing and submittal review. All necessary construction progress inspections, including punch-list and occupancy inspections. As-built drawings and specifications showing all changes made during construction. Travel costs and all other miscellaneous expenses. Include mechanical engineering for new plumbing work. CONSTRUCTION: General Requirements 12

13 Keep a current and up to date copy of the construction documents on the job site marked with red-lines for all changes that occur during the work. Provide all required shop drawings and submittals, and keep a copy of all approved shop drawings and submittals in the job site office. Turn over all approved files as well as all appropriate operation and maintenance manuals to the Owner upon completion of the project. Provide all necessary samples and test panels. Provide all necessary temporary utility hook-ups. Provide all necessary permits and pay for all deputy inspection fees. Pay all applicable taxes on the work. Standard job site work hours are 8 AM to 5 PM. Permission to work at the site during any periods other than standard work hours must be approved in advance, in writing. HEPA filters and infection control procedures as required by the facility. Maintain negative pressure in the construction area as required by the facility. Provide for daily broom cleaning of the job site and debris removal and appropriate disposal. Use of walk off mats as required by the facility. The entire job site shall be thoroughly cleaned upon completion of the work, prior to turnover to the customer. Compliance with the Owner's security regulations and dress codes is included. Existing Conditions The installation of code compliant temporary partitions to secure areas, control dust, protect adjacent areas and equipment as required are included. Include removal and disposal of all existing floors, case work, ceiling tiles & grid and electrical components as required to accommodate the new work. Include removal of the existing wing wall. Lead drywall and lead window. Provide labor and equipment to demo a 36 wide opening from the dark room into the Rad room. Include saw cutting in dark room to tap into the waste line for relocated sink. Concrete Patch concrete slabs at all cuts, cores, and areas affected by the construction. Include patching and infilling concrete where removed for plumbing work in the dark room. Include rebar and dowels as required. Metals Furnish labor and material to frame new control wall per space plan LAYOUT X-RAY ROOM (see enclosed layout drawing). Furnish labor and material to frame in existing processor opening at existing dark room. Furnish and install 3 x3 x ¼ steel angles for attachment of Philips Essenta DR equipment in accordance with structural requirements. Frame new cased opening from dark room (Room 112C) into Radiology room (112B). Furnish labor and material to frame in new 24 x24 view window at dark room (non lead). 13

14 Include patching wall at dark room where new plumbing is installed. Wood, Plastics and Composites Provide and install 9 lineal feet of plastic laminate counter at control (L-Shape) Include 16 tall plexiglass divider at sink per drawing. 6 Lineal feet of work counter in dark room opposite the control counter. Openings Existing doors to remain in existing location. Include new lever hardware at (2) doors in the X-Ray room to meet ADA requirements. Furnish and install (1) 24 x 24 standard glass window with timely frame at the dark room (facing vestibule area). Finishes All existing drywall and/or plaster construction disturbed by the work shall be patched, repaired or replaced as required with materials and construction type compatible with the existing construction. Provide new 2' x 4 suspended ceiling system in the X-Ray room only. Ceiling tiles to match existing area. (NOTE: Ceiling to be raised to 9-3 from 9-0 ) All walls shall be primed coat painted, and (none) of the walls shall receive commercial grade vinyl wall covering of the owners choosing from samples submitted by the material supplier -the balance of the walls shall be final coat painted in no more than two different colors as selected by the owner from samples submitted by the material supplier. Furnish and install new 1/8th inch thick commercial grade non-static 12 x12 VCT to meet facility standards. Include patching and repairs at areas affected by our work. Furnish new VCT in the dark room with 4 topset base. All materials to be as selected by the owner from samples provided by the material supplier. All door frames shall be painted, all doors shall be plastic laminate to meet facility standards. All existing finishes disturbed by the work shall be patched, repaired or replaced as required with materials and construction type compatible with the existing construction. Equipment Furnish labor and equipment to remove the existing X-Ray equipment, film developer and components. Special Construction FLOOR PLATES: Installation of the Philips supplied equipment base plate(s) is included (If applicable) RADIATION SHIELDING -X-RAY: Supply and install all radiation shielding as required. Include patching and repair of existing lead shielding where affected by our project. Include new 24 x24 lead view window (Per X-RAY rooms). FLOOD SAFETY PAN: Install flood safety pan above x-ray machine with appropriate drainage to safeguard against water damage in the event of a roof or plumbing leak. Plumbing 14

15 Furnish labor and material to re-plumb the existing dark room sink including relocating the waste, hot and cold water. Include re-routing the vent line and tieing in above ceiling to same point of connection. Furnish labor and material to cap off the existing floor drain and floor sink in the dark room. Install ESD ( Tile in the X-ray & Dark Room for proper Grounding of floor to prevent static. Keep existing small Dental film developer in the Dark Room. Heating Ventilating and Air Conditioning Furnish labor and material to re-balance the existing air serving the room. Include reconnecting existing grilles to new t-bar ceiling in both the RAD room and the Dark room. Include test and balance of the HVAC system(s) affected by the work shall be performed by a qualified independent testing agency certified for such work. Any rework required to bring the HVAC system (or portion of the system affected by the work) to within design specifications is included. Provide copies of all test reports is needed. Electrical Existing main Circuit Breaker enclosure to remain is existing location. The power feed shall connect to the existing 100 AMP breaker located in the existing X-Ray room and must meet Philips Essenta DR imaging system requirements for power quality as specified in the Philips site planning documents. Include safe off for removal of existing X-Ray equipment currently in the control room. Include installation of all required conduits, raceway and boxes per equipment requirements. All raceway to be surface mounted. All required circuit breakers, taps, sub-panels, etc. as required are included. Installation of misc. Philips equipment related items as specified on the referenced Philips Site Planning Department documents is included. Furnish and install (4) new 2x4 florescent light fixtures in the X-Ray room. Furnish and install (1) new florescent light fixtures in the dark room and (3) H7 incandescent light fixtures in the dark room. Include removal of old fixtures as required. Communications TELEPHONE SYSTEMS: Provide all conduit, junction boxes, back-boxes, mounting boards, cover plates and pull boxes and pull strings for owners telephone system to meet facility standards (system and installation by others). Rough-ins for telephone outlets to be in the following rooms: Control desk. COMPUTER NETWORK SYSTEMS: Provide all conduit, junction boxes and pull strings for owner s computer network system to meet facility standards (system and installation by others).
