3 The Conservatory School for the Performing Arts, LLC Release and Waiver Liability, Assumption of Risk, and Indemnity Agreement REGISTRATION IS INCOMPLETE WITHOUT SIGNATURE AND MUST BE COMPLETED BEFORE THE AUDITION I understand and agree that in participating in any classes, programs, workshops, camps, events, or activities there is a possibility of physical illness, bodily injury, disability, paralysis, or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of The Conservatory School for the Performing Arts, LLC classes, programs, workshops, camps, events, or activities. I also exempt, release, and indemnify The Conservatory School for the Performing Arts, LLC, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by The Conservatory School for the Performing Arts, LLC. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold The Conservatory School for the Performing Arts, LLC its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. Permission is granted to The Conservatory School for the Performing Arts, LLC to use photographs and videos of students for publicity purposes. 4 Pre-Professional Musical Theatre Audition Dress Code and Regulations Female Performers: Leotard, black jazz pants/ black dance shorts/ tights, hair neat off the face (ie: Bun, half up half down, ponytail.) Bring all dance shoes with you. Male Performers: Solid color t-shirt (no logos), Black jazz pants/black dance shorts. Bring all dance shoes with you. All Performers: No jewelry (small earrings are fine.) Audition Schedule 5:00pm-5:15pm: Check in 5:30pm-6:30pm: Closed auditions Promptness: All performers must arrive from 5:00pm-5:15pm to check in, stretch, and prepare. Closed Audition Process There will be no public viewing of auditions. Results will be posted 1-2 weeks after the audition. An will be sent to notify you about the result posting date. Audition Requirements 1) Completion of audition registration 2) Proper audition dress code 3) Students must display strong work ethic, dedication, good sportsmanship, and adhere to the CSPA Code of Conduct 4) No food or drinks allowed in the dance room 5) Serious submissions only 6) Remember to have fun! Registration Checklist 1) All pages and sections of the registration form are completely filled out 2) I have signed & dated the release & waiver form 3) Audition fees are paid when the audition registration form is submitted 4) Registration can be mailed with (cash/check) or hand submitted by Monday, June 5, 2017

5 5) I have submitted the Parental Acknowledgment Form with Registration 6) I have fully read the audition dress code & regulation form Summer Stock Rehearsal Tuition & Fees One Show Rehearsal Fee: Includes 2 hours of rehearsal per week - $ Tuition Break Down $ divided by 6 = $18.66 divided by 2 hours = $9.16 per hour Two Show Rehearsal Fee: Includes 4 hours of rehearsals per week - $ Tuition Break Down: $ divided by 6 = $35.83 divided by 4 hours = $8.95 per hour. Three Show Rehearsal Feel: Includes 6 hours of rehearsals per week - $ Tuition Break Down: $ divided by 6 = $53.33 divided by 6 hours = $8.88 per hour. Rehearsal Tuition Due by June, 17, 2017 Costume Fees: Costumes will have to be ordered or professionally made per show. Costume fees can range from $75.00 to $ per costume. Please plan accordingly. Tickets will go on sale approximately 1-2 weeks before opening night. Ticket prices TBD. Summer Stock Rehearsals Students must be present to all rehearsals, dress rehearsals, and show date to participate in the Summer Stock Show. Rehearsals will start the week of July 17th and end the week of August 25th. Show date will be on the August 26th or August 27th. Please plan accordingly. Students must be present to all rehearsals to participate in Summer Stock. Rehearsals will take place on Friday s 9:30am-3:30pm (July 17 through August 25) Annie 9:30am-11:30am Charlie and the Chocolate Factory 11:30am-1:30pm Waitress 1:30pm-3:30pm

6 Parental Acknowledgment Form I understand that audition fees are non-refundable I understand that my child s rehearsal tuition is due by June, 17, 2017 I understand that all judges critics are property of the studio and may not be shared I understand that the Director s and Judge s decisions are final I understand that the audition is closed to the public I understand that The Conservatory has a zero tolerance policy of harassment and/or bullying via in person, in writing, via social media to CSPA staff, parents, and students. I understand that my child s spot in the Summer Stock Show may be terminated by poor behavior from myself and/or child. I understand that my child will have to attend all rehearsals, dress rehearsals, and show(s.) Failure to show to the listed or additional events may result in termination of my child s role. I understand that costume fees will apply. Costume prices will range from $75.00-$ per costume.
