6 MUSIC DISCOVERIES 2018 WAIVER AND AGREEMENTS Please read the following waiver carefully. It includes release of liability and waiver of legal rights, and deprives you of the ability to sue certain parties. By signing below, you acknowledge that you have both read and understood all text presented to you as part of the registration process. Waiver I agree that my child will follow all reasonable instructions and directions of the leaders and instructors duly appointed by the University of Victoria Student Music Educators' Association (UVSMEA) in connection with the operation of Music Discoveries. There are many possible risks associated with participation in Music Discoveries such as: - Allergic reactions to food served at the camp; - Fighting with other students; - Falling while carrying a musical instrument to name but a few. I hereby release, remise, and forever discharge the UVSMEA, the University of Victoria, the University of Victoria Students' Society, their agents, and their volunteers of and from all manner of actions, claims, and demands of whatever nature which result from any loss, injury, or expense sustained, arising out of or in any way connected with participation in any program or attendance at a location operated by the UVSMEA. I FREELY ACCEPT AND FULLY ASSUME ALL RISKS, DAMAGES AND HAZARDS AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, PROPERTY DAMAGE AND LOSS TO MYSELF OR MY CHILD. BY SIGNING THIS FORM OR BY CLICKING "I ACCEPT" BELOW, I AGREE TO WAIVE ALL CLAIMS DUE TO ANY CAUSE WHATSOEVER, INCLUDING NEGLIGENCE, BREACH OF CONTRACT OR BREACH OF ANY STATUTORY OR OTHER DUTY OF CARE. I UNDERSTAND THAT THE UVSMEA, UVSS, UVic, THE MUSIC DISCOVERIES COORDINATORS AND THEIR AGENTS OR VOLUNTEERS ARE NOT RESPONSIBLE FOR PROVIDING ME WITH ANY REFUND FOR FEES PAID FOR MUSIC DISCOVERIES I authorize the UVSMEA to use any photographs, video or audio recordings taken of my child while participating in Music Discoveries 2018 to be used in future Music Discoveries and UVSMEA brochures and promotional material/media for the sole purpose of improving Music Education in Victoria. By signing, I am aware of and agree that my child will comply with the rules regarding no alcohol, no recreational drugs, no violence, and no sexual activity at Music Discoveries ACKNOWLEDGEMENT I acknowledge that I have read this agreement, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, executors, administrators and representatives, in the event of my death or incapacity.
