1 Clarinet and Klezmer in the Galilee August 16-24, 2011 Safed, Israel Application Form Thank you for your interest in Clarinet and Klezmer in the Galilee - a weeklong program of master classes and performances in the Old City of Safed! All musicians classical, jazz and klezmer clarinetists and klezmer/jazz musicians in all instruments - age 16 and over are warmly invited to join the program. Participants will receive both master classes and lessons during the course of the program. Classes will take place in different locations around the city, including historical synagogues. An organized tour of Safed and environs is also included in the program. We hope you will join us for what promises to be a musically historical event in the magical city of Safed. 2 Financial Statement : A limited number of scholarships are available based on fnancial need. If you wish to be considered for a scholarship, please provide a fnancial statement on a separate page, including your and your parents (if relevant) income, its sources, and what you are able to provide towards the cost of attending the program. This fnancial statement must accompany your application in order for a scholarship (or partial scholarship) request to be considered. Personal Photos: Please attach two passport photos, which will be used for publicity purposes. Parental Approval (for candidates under 18) : Signed by parents / guardian. A check for advance payment or an approval of a bank transfer (swift) of the advance payment. An application which lack the above mentioned documents will not be examined Admission General Terms of Participation form Every request for admission submitted according to the required terms will be examined by the organizers. The organizers have the exclusive discretion regarding admission to the program. By signing this form candidates/participants agree to waive andqor withdraw any demand and/or claim regarding admission to the program. Period of the Program Every participant should arrive at Safed according to a prior notice by the organizers on Monday 16/08/2011 by11:00 Organizers can extend the program period untill the August 24 of 2011.

3 Tuition Registration fee for the program is 50. (non refundable). Accommodations in a single room will be available on limited basis and for a supplemental fee. 3 meals a day Breakfast will be served by the accommodation place, Lunch + Supper will be served by the organizers. All instruction and organized outings. A registration fee of 50 (non-refundable) in the form of a check or money order, and accompanied by this application form, should be sent to the address below by May 15,2011. Insurance We strongly recommend all participants to have a health insurance as well as instrument insurance for the period of the festival. Please bring a copy of your insurance on the day of arrival. The festival management will not be responsible for any instruments or medical expenses that might occur during the festival. Arrival and Departure Participants should plan to arrive in Safed on, 16/08/2011 by11:00. The organizers can provide assistance in arranging

4 transportation to and from Ben-Gurion Airport, although the cost is not included in the tuition.
