1 Trainee Registration: On opening the url the fourth menu item in the menu bar at the top is Trainee Registration menu item. Clicking this displays the following screen: The Select Language drop down gives the user the option to choose either English or Kannada as the preferred language. English has been chosen for this example. The sections along with the fields are described below: Personal Details: 1. First Name*: This is a text field to enter alphabets only. The entry can be cleared by clicking on the x mark at the far right of the field. 2. Last Name: This a Non Mandatory text field to enter alphabets only. The entry can be cleared by clicking on the x mark at the far right of the field. 3. Phone Number*: Mandatory field to enter the 10 digit phone number. Only numbers are allowed and have to be 10 digits in length. The entry can be cleared by clicking on the x mark at the far right of the field. 4. *: This is a Mandatory text field to enter a valid address. This has to be in the format xxxx@yyy.zzz. The entry can be cleared by clicking on the x mark at the far right of the field. 5. Date Of Birth*: This is a Mandatory field wherein the date of Birth has to entered using the Calendar control that is displayed. 6. Aadhaar Number*: This is a Mandatory filed wherein the user must enter the 12 digit Aadhaar number. Only numbers are allowed in this field. The entry can be cleared by clicking on the x mark at the far right of the field. 7. Gender*: This is a Mandatory field of type radio button to select the Gender of the Trainee. 8. Marital Status*: This is a Mandatory field of type radio button to select the marital status.

2 9. Physically Challenged: This is a Non Mandatory radio button selection. Selecting Yes displays a new field titled Physically Challenged Types. a. Physically Challenged Types*: This is a Mandatory drop down list wherein the user can select one or more of the following types: Visual Locomotive Hearing Other 10. Religion: This is a Non Mandatory drop down list to select the religion. 11. Category*: This is a Mandatory field of type radio button. Selecting OBC displays a new drop down titled OBC Sub Category. a. OBC Sub Category*: This is a Mandatory drop down to select the OBC sub category. The screen shot below shows the fields with filled in values as example: Father/Mother/Guardian Details In this section, the user has to select one of the radio buttons for Father or Mother or Guardian and the Salutation from the drop down. 1. First Name*:This is a Mandatory text field which accepts only alphabets. The entry can be cleared by clicking on the x mark at the far right of the field.

3 2. Last Name*: This is a Mandatory text field which accepts only alphabets. The entry can be cleared by clicking on the x mark at the far right of the field. The screen shot below shows the fields with the filled in values as example: Current Address: 1. Location*: This is a Mandatory radio button selection. Selecting Urban displays a text field with label City. Selecting Rural displays a text field with the label Village. 2. Street Address line1*: This is a Mandatory field to enter address. The entry can be cleared by clicking on the x mark at the far right of the field. 3. Street Address line2*: This is a Mandatory field to enter address. The entry can be cleared by clicking on the x mark at the far right of the field. 4. State*: This is a Mandatory drop down list to select the State. This selection determines the entries in the District Drop down which is the next field. 5. District*: This is a Mandatory drop down whose entries depend on the State selected. The selection here determines the entries in the Taluk Drop down, which is the next field. 6. Taluk: This is a Non Mandatory drop down list to select Taluk. 7. City*: This is a Mandatory text field to enter the city. As mentioned previously, this filed will be labelled as Village if the Location is chosen as Rural. The entry can be cleared by clicking on the x mark at the far right of the field. 8. Postal/Zip Code*: This is a Mandatory text field to enter the 6 digit zip code. The entry can be cleared by clicking on the x mark at the far right of the field. Checking the Same as Current Address copies the Current address values to the Permanent address fields, which is the next section, and has the same fields as the Current address section.

4 The screen below shows the Current address section with the filled in values for example.

5 Education: 1. Education*: This is a Mandatory drop down list to select the educational qualification. This selection determines the list items in the Stream drop down, which is the next field. 2. Stream*: This is a Mandatory drop down to select the Stream and the selection here determines the list items in the Subject drop down, which is the next field. 3. Subject*: This is a Mandatory drop down to select the subject. 4. Year of Passing*: Mandatory text field to enter the year of passing. 5. Skills Sought: Non Mandatory Drop down list where in the user can one or more items by clicking on the check box(es) in the drop down. One can select a maximum of 5 skills only. 6. Preferred Duration of Training Required*: Mandatory drop down to select the preferred duration of training. 7. Willing to Take Apprenticeship: Non Mandatory radio button selection. 8. Have you been previously employed: Non Mandatory radio button selection. Selecting Yes displays the following new fields: Total Years of Work Experience*: Mandatory text field to enter 2 digit number. Name of Last Employer*: Mandatory field to enter the name of the last employer. Last Designation*: Mandatory filed to enter the last designation held. Address of Last Employer*: Mandatory text field to enter the address. Last Drawn Salary in Rs*: Mandatory drop down to select the salary range. 9. Are you willing to Migrate?: Non Mandatory radio button selection. Selecting Yes displays the following new Mandatory field: a. Migration Area*:Mandatory drop down, where in the user can select one or more items by checking the checkbox(es) in the drop down. 10. Expected Salary Outside District in Rs*: Mandatory drop down to select the salary range. 11. Expected Salary Within District in Rs*: Mandatory drop down to select the salary range. The code displayed in the image needs to be entered in the field Enter the Code Here. The Aadhar Consent check box must be selected to enable the Submit button.

7 Clicking on the submit button displays the dialog box as shown below Clicking on Yes completes the Registration process and the Serial Number is displayed as shown below. This concludes the Trainee Registration Process.
