2016-2017 Music Boosters Scholarship Information Sheet The Monroe-Woodbury Music Boosters Scholarship program seeks to reward those select students in grades 5-12, who demonstrate outstanding musicianship and success in their dedication to music. The student s confidence and passion for learning music will be apparent in Chorus, Band, Orchestra, or any combination of these ensembles. PLEASE NOTE: If you received a scholarship award last year, you are not eligible to apply this year unless you are in 12th Grade. ~Print clearly on your application. All essays must be typed! ~Please include an email address for communications. ~The application requires a Teacher Recommendation Form from a Monroe-Woodbury Music teacher. You may provide up to 2 teacher recommendations, but one of them must be from a Monroe-Woodbury Music teacher. The other may be from a private teacher. *After filling out your name, school, grade and teacher s name, please give each teacher a separate form. The teacher(s) will then complete the form and send it back to us. If you are asking a private teacher to complete a form for you, please provide them with a stamped envelope addressed to: Monroe-Woodbury Music Boosters, PO Box 867, Harriman, NY 10926. * Please do NOT ask a teacher to fill out a recommendation form until you are sure you will be submitting an application. ~Interviews - All interviews will be conducted in person on April 3, 2017 at Central Valley Elementary School. There are no alternate interview times. If you are not interviewed at that time, you cannot be considered for a scholarship. No exceptions will be made. Interviews are typically conducted between 6:30 pm and 8:30 pm. Further information regarding interview times will be sent out at the end of March. *If you do not attend your scheduled interview, your application will not be considered. Late arrivals for interviews cannot be guaranteed an appointment. ~Scholarships will be presented to winners during their school s spring concerts. Scholarships will be cash awards, amounts to be determined depending on the number of applicants. ~ Please see the checklist on the back of this information sheet to make sure all important steps have been taken, and you are ready to submit the application.

Music Boosters Scholarship Checklist Please make sure every item on the checklist is completed before you turn in your application. Every part of the application is fully complete and legible. Your name and grade are on each sheet. All teacher recommendations are distributed.
