24 Important notce for prospectve hearng ad users (US only) Good health practce requres that a person wth a hearng loss have a medcal evaluaton by a lcensed physcan (preferably a physcan who specalzes n dseases of the ear) before purchasng a hearng ad. Lcensed physcans who specalze n dseases of the ear are often referred to as otolaryngologsts, otologsts or otorhnolaryngologsts. The purpose of medcal evaluaton s to assure that all medcally treatable condtons that may affect hearng are dentfed and treated before the hearng ad s purchased. Followng the medcal evaluaton, the physcan wll gve you a wrtten statement that states that your hearng loss has been medcally evaluated and that you may be consdered a canddate for a hearng ad. The physcan wll refer you to an audologst or a hearng care professonal, as approprate, for a hearng ad evaluaton. The audologst or hearng care professonal wll conduct a hearng ad evaluaton to assess your ablty to hear wth and wthout a hearng ad. The hearng ad evaluaton wll enable the audologst or hearng care professonal to select and ft a hearng ad to your ndvdual needs. If you have reservatons about your ablty to adapt to amplfcaton, you should nqure about the avalablty of a tral-rental or purchase-opton program. Many hearng care professonals now offer programs that permt you to wear a hearng ad for a perod of tme for a nomnal fee after whch you may decde f you want to purchase the hearng ad. Federal law restrcts the sale of hearng ads to those ndvduals who have obtaned a medcal evaluaton 51 400973011US-18.08-Rev.A.ndd 51 01-08-2018 14:18:34
