OKLAHOMA 4-H REPORT FORM NAME COUNTY ENTRY CATEGORY Level 1 Level 2 Hall of Fame 4-H PROJECT AREA (ENTER 1 PROJECT FOR LEVEL 1 OR 2) AGE/GRADE AS OF SEPTEMBER 1, 2018 LEVEL I & II PROJECT CHECK LIST 4-H member will complete check list below to make sure they have formatted their entry correctly. A penalty of 5 points per item will be assessed to the record book score if not correct! Name, county and program area on a label in upper right corner of cover or folder. Oklahoma Report Form Page 1 all information complete, including all signatures Section 1A maximum of 2 pages Section 1B 1 page Section II maximum of 2 pages Section III maximum of 2 pages Section IV & V maximum of ½ page for each section on same page One copy of the 4-H story bound in the book. Story is no more than 6 typed or printed pages on 8 1/2" x 11" white paper, printed on one side of the paper, and double spaced. Story margins are: Left: 1.25 inches; Right, top and bottom:.5 inch. There is no need to list name, county and project on the top right of the story. No more than three pages of photos. Photos should have captions. Photo page margins are: Left: 1.25 inches; Right, top and bottom:.5 inch. PHOTOGRAPHY Project Book may have 10 additional pages of pictures which can be 8 X 10 prints or smaller of examples of photography work and captions Font size for Oklahoma Report Form, Story and Photo captions must be a minimum of 12 point font. Approved fonts include: Times New Roman, Arial, Cambria, or Calibri (THIS ITEM IS AN AUTOMATIC DISQUALIFICATION!)
