ELECTRICAL SAFETY INSPECTION REPORT A PLUS INDUSTRIES LIMITED. Plot-28, Milk Vita Rd., Section-07, Mirpur, Dhaka Factory List: 1. A Plus Industries Ltd. Inspected by: Yoon Report Generated by: Nezar Inspected on March 10, 2014

SUMMARY A plus Industries Limited occupies a six-storied building. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. A PLUS INDUSTRIES LTD Page 2 of 10

FINDINGS AND RECOMMENDATIONS Finding #: E- 1 CABLE & CABLE SUPPORTS Cables terminating from wall are wrapped with combustible material. Remove all wrapped combustible material from cable. Cables/wirings passing through permanent wall must be protected installing pipes and remaining gaps must be sealed with fire resistant materials. Cable tray/raceway shall be installed for the support of the cable throughout its length. Cable terminating to room through wall. A PLUS INDUSTRIES LTD Page 3 of 10

Finding #: E- 2 High earth loop impedance measured. Check for loose earthing-connection and take necessary action accordingly. Measuring earth loop impedance. Finding #: E- 3 CABLE & CABLE SUPPORTS Cables terminating at panel are not firmly fixed. Install cable tray or riser to support the cables entering and leaving the electrical panel to reduce cable strain on cable termination point. Provide cable gland according to the respective cable size for cable entry and exit so that the cables are not stressed on the sharp edges of the hole of panels. Panel box fixed on wall. Finding #: E- 4 SERVICE CABLE Service cable directly laid on floor without any protection. Install the cables tray or duct with cover (metallic) for the protection of the cable laid on floor. Ensure the cables are tightly latched inside the ladder/tray and provide covers made of non-combustible material preferably metallic sheet to protect the cables insulation from any physical damage as well as prevent the ingress of debris, dust and lint. Finding #: E- 5 CABLE & CABLE SUPPORTS Service cables terminating from wall is not supported. Cables must be protected and supported and installed through safe and prescribed routes. Cables laid on external walls of the building must be supported on covered ladders and cable trays in complete length of cables. Seal all the penetrations using appropriate fire rated material and ensure the cable insulation does not get damaged during sealing work. Cables are laid on floor. Finding #: E- 6 TRANSFORMER ROOM Transformer room not maintained regularly. Transformer room must be regularly cleaned and maintained as part of the routine maintenance. Disconnect the transformer from the electrical system before maintenance work and cleaning. Transformer room Finding #: E- 7 SERVICE CABLE Excess service cable coiled on pole not supported. Excess length of service cables must be trimmed and install a vertical cable tray or ladder with protective cover to support and protect the HT cable. Finding #: E- 8 TRANSFORMER ROOM Silica gel in transformer breather, discolored. Shut down the transformer and replace the silica gel or perform maintenance to remove moisture from it. Consult with transformer servicing company before performing the task. Establish a routine maintenance & inspection program for transformer as well as all other electrical equipment to ensure any future repetition of the occurrence. Breather of transformer. Finding #: E- 9 TRANSFORMER ROOM Oil cup below transformer breather is empty. Fill the Breather oil cup with transformer oil up to the required level as instructed by the manufacturer. Consult with transformer servicing company before performing the task. Establish a routine maintenance & inspection program for transformer as well as all other electrical equipment to ensure any future repetition of the occurrence. Breather of transformer. Finding #: E- 10 Gland holes in cable base plates left open. Provide cable gland at the base plate of panels according to the respective cable size for cable entry and exit so that the cables are not stressed on the sharp edges of the hole of panels. Remediation Timeframe: WITHIN 6 MONTHS Finding #: E- 11 Excessive heating of cables inside panel. Arrange periodic inspection to identify the overloading, loose connection, unbalanced load which may cause the excessive heat-rise and take action accordingly. Measuring cable temperature. Finding #: E- 12 Panel doors not connected with earth bond. Provide earth connection for body and doors of metallic distribution boards using green cables preferably braid so that the metallic door remains at zero potential all the time. Panel board without earth bonding. Finding #: E- 13 GENERATOR ROOM Generator battery placed on the concrete floor. Remove all the combustible materials, kept near electrical equipment. Generator Battery must be place done the battery stand made of noncombustible material (steel fabricated, acid proof). Generator battery. A PLUS INDUSTRIES LTD Page 7 of 10

Finding #: E- 14 CABLE & CABLE SUPPORTS PVC conduit broken/damaged. Broken conduit must be replaced with new standard PVC conduit. PVC conduit damaged/broken. Finding #: E- 15 Barrier/separators between different phases are not installed. Provide phase separators between poles of MCCB made of non combustible materials preferably use rubber having enough dielectric strength to insulate phases from each other. MCCB inside panel board. Finding #: E- 16 TRANSFORMER ROOM Excessive dust and/or lint deposit on the transformer. Establish a routine cleaning program to keep neat and clean the transformer room. Finding #: E- 17 Multiple cables terminating to MCCB in panel. Multiple cables connecting at a MCCB terminal must be removed. Individual circuit breaker must used for each load according to the respective cable-size. Wires connected with MCCB. Remove all the multiple connections made at a single point of bus bar and connect individual branch cables to individual points on bus bar using individual lug according to the respective cable size. Wooden board/combustible material should not be used in any electrical wiring system. Use any combustible material to mount any electrical devise. Wooden board fixed with PVC sanitation pipe.
