Attached are contracts/release forms for the 2016-17 school year. These forms need to be read, signed and returned to Mrs. DeMattos by Monday, August 22 nd, 2016. Failure to turn in forms by 8/22/16 will not only affect your grade, but your ability to perform and/or remain in Gospel Choir. You should have already read the SHS Choral Constitution which was emailed through Skyward and available for download on the school website. The required student fee for Gospel Choir is listed below. The payment due dates are as follows: Tuesday, September 15 th, 2016 $42.00 Tuesday, October 25 th, 2016 $42.00 Tuesday, November 2 rd, 2016 $26.00 (if you want to attend the banquet) Gospel Uniform Yearbook Banquet Music Clinician Supplies Total Total Without Banquet Choir Rental (Optional) Dues $10 $2 $26 $10 $55 $7 $110 $84 Student accounts must be paid in full by Friday, Nov. 11 th, 2016. Students whose accounts are not up-to-date will not be allowed to perform at Disney Candlelight or Disney Marathon performances. The Classic Cookie Fundraiser starts Sep. 26 th and goes until Oct. 17 th. Money raised through Classic Cookie (40%) goes directly into student accounts and can replace direct payments. Students are encouraged to participate in chorus fundraisers to pay off their accounts. Students in financial need must communicate to Ms. Parkhurst via parent email and scholarships will be awarded on a case-by-case basis. Please feel free to contact me with questions or concerns. If I am unavailable, you can always contact the Choral Assistant, Loretta DeMattos. She handles student accounts and supports the program at performances and during the school day. Please watch for emails from myself or Mrs. DeMattos to keep you updated on the latest activities. 2 SHS Choral Contract Gospel Choir Uniform requirements Maintaining a professional appearance is a part of your grade. Students failing to meet the uniform requirements will not be allowed to perform. Gospel choir robes are purchased by the program and students pay a portion of their dues for costume upkeep, cleaning, repairs/alterations, etc. There are also additional items (mentioned in the audition contract) that students are required to purchase individually and use throughout the school year. In many cases, students can purchase these items directly through the program. Students are expected to wash and maintain these items. I ve included a bulleted list of these items below. All Gospel members need: Black Chorus Tee-Shirt Long, Solid black dress pants (No capri, jeans, leggings/yoga pants, sweat pants or skinny pants) Plain Black Socks or stockings Solid Black, Closed Toe/Back, Dress Shoes (no tennis shoes, combat boots, flip flops or sandals, etc.) Additional performance attire requirements: HAIR: All performers are required to have natural hair color for any of their performances/auditions. For performances, hair color/styles must not be distracting and must not be in the face of the performer. (Ex. Mohawks, spikes, & colors not of the natural color wheel) JEWELRY: For Disney NO JEWELRY may be worn at any performance. For in-school performances, students may only wear rings and stud earrings. Dangling earrings, necklaces, bracelets, or any jewelry/accessory that is distracting may not be worn. FINGERNAIL POLISH: For all performances, fingernails must be natural or nude/clear. Bright colors, long nails, or distracting manicures are not allowed. Students wishing to audition for Disney s Candlelight Processional or Half Marathons have a separate dress code, which includes: White Chorus Polo - $16 purchased through SHS Choral Department Long, Solid black dress pants (No capri, jeans, leggings/yoga pants, sweat pants or skinny pants) Plain Black Socks Solid Black, Closed Toe/Back, Dress Shoes (no tennis shoes, combat boots, or flip flops, sandals, etc.

3 SHS Choral Contract Return to Mrs. DeMattos signed! GOSPEL CHOIR AGREEMENT I,, have read the 2016-2017 Choral Constitution and Gospel Choir Contract and understand all that is required of me. It is my responsibility to advise my parent/guardian of the information put forward by the SHS Choral Department and return all paperwork by the assigned deadline. I solemnly promise to follow the rules and regulations as set forth in the constitution at all times during the upcoming academic year. I furthermore understand that I am to maintain a minimum 2.0 Grade Point Average per nine weeks, adhere stringently to SHS attendance and behavior policies and abide by the No Tolerance regulations as set forth by the School Board of Seminole County. I recognize that I am a role model for other Seminole High School students and agree to conduct myself, at all times, in a manner that will make my parents, teachers, administrators and community creditable and proud. I/We am/are the parent(s) or legal guardian(s) of the above named choral member and by signing this contract, I/We am/are giving my/our permission for my child to participate in the above named choral ensemble for the 2016-2017 school year. In addition, my/our child has my/our approval to participate in all fund-raisers understanding that, if my/our child s account is not paid, or if funds raised are not turned in, I/we will be held responsible. It is my/our responsibility to have my/our child communicate with me/us on the availability of fundraisers, planned trips and competition(s).
