Title is double spaced if it wraps more than one line (i.e. one space between each line) Student s full name is the same under the title and after the copyright symbol Degree is listed correctly and spelled out (Doctor of Philosophy) Department and College are listed and spelled out University of South Florida St. is spelled Petersburg out is spelled out Defense/Approval date is listed (Date cannot be later than date of submission) (Co-)Major Professor(s) and committee members full names and degrees credentials (i.e., Ph.D.) are listed Keywords are listed (minimum of 5 words or phrases that are not used in the title) One line space between Keywords and Copyright Copyright and Copyright symbol is listed and the year matches the year of the final submission (i.e. Copyright year, Author) No page number is included on the Title Page

ABSTRACT PAGE CHECKLIST The Abstract is double spaced Abstract header is spaced ~2 from top of paper (1 below top margin) The style and location of the Abstract header matches those of all other major headers (i.e. all capitals or upper and lower case; centered or left aligned) The page number on the Abstract is typed in LOWERCASE ROMAN NUMERALS (ii, iii, iv, etc.) and numbered consecutively from the Table of Contents The Abstract page number is positioned in the same location as page numbers in the remainder of the manuscript If the Abstract exceeds one page, the text of the continuation page is spaced at the top margin, 1 below the top of the paper

BODY OF THE MANUSCRIPT CHECKLIST The body of the text is double spaced All Major Headers are spaced ~2 from top of paper (1 below top margin) The style and location of all major headers is the same (Options include: All capitals or upper and lower case; centered or left aligned) Continuation pages are flush with the top page margin, 1 below the top of the paper All subheadings are in the same style and location throughout the manuscript (i.e. upper and lower case left aligned; 0.5 indent and in bold, etc.) No isolated lines of text or subheads at the end of pages (widows) No single lines of text at the end of paragraphs at the start of new pages (orphans) All Tables are numbered consecutively and typed in Arabic numerals using a consistent style Type the tile of the each Table ABOVE the Table All Tables are inserted in the manuscript in a consistent manner (within the text, on a free standing page or compiled at the end of each chapter) All Figures are numbered consecutively typed in Arabic numerals and using a consistent style All Figures have a title and legend positioned BELOW the figure All Figures are inserted in the manuscript in a consistent manner (within the text, on a free standing page or compiled at the end of each chapter) All Figures are high resolution. If text is included in figure, be sure that it is legible The page numbers are typed in ARABIC NUMERALS (1, 2, 3, 4 etc.) and numbered consecutively beginning on the first page of Chapter 1 and ending at the Appendices Page numbers are positioned in the same location throughout the entire manuscript

REFERENCES CHECKLIST IF REFERENCES ARE MAJOR SECTION Font is the same style as the remainder of the manuscript References header is spaced ~2 from top of paper (1 below top margin) The style and location of the References header matches those of all other major headers (Options include: All capitals or upper and lower case; centered or left aligned) Continuation pages are flush with the top page margin, 1 below the top of the paper References are listed in one consistent style The page numbers are typed in ARABIC NUMERALS (99, 100, 101 etc.) and numbered consecutively from the body of the manuscript Location of the References page numbers are the same as others throughout the manuscript IF REFERENCES ARE SUBSECTION OF A CHAPTER Font is the same style as the remainder of the manuscript Heading formatted as other secondary headers Section DOES NOT begin at the top of a page unless the previous section ends at the bottom of the previous page Continuation pages are flush with the top page margin, 1 below the top of the paper References are listed in one consistent style The page numbers are typed in ARABIC NUMERALS (99, 100, 101 etc.) and numbered consecutively from the body of the manuscript Location of the References page numbers are the same as others throughout the manuscript

APPENDICES CHECKLIST **This section is not required** * The font size and style of the Appendix sections may be different from that within the body of the manuscript but must still meet the margin requirements For written text, the font should be the same style as the rest of manuscript (font size can be reduced) If used, the Appendices header is a major header and is located ~2 from top of paper (1 below top margin) on the Appendices title page in the same style as other major headers If the Appendices is a single chapter, the Appendix subsections headers are indented just as other 2 nd level headers in the Table of Contents The Appendix subsections headers are flush with the top page margin, 1 below the top of the paper in the same style and location as other 2 nd level headers If all of the Appendix sections are an independent chapter, there will be no Appendices title page All Appendix sections are major headers and will be located ~2 from top of paper (1 below top margin) on the Appendices title page in the same style as other major headers Appendices sections are numbered using ARABIC NUMERALS (99, 100, 101 etc.) and numbered consecutively from the body of the manuscript Published manuscripts are compiled within the last Appendix chapter and do not need to be numbered

ABOUT THE AUTHOR CHECKLIST **This page is not required** The About the Author section is double spaced About the Author header is spaced ~2 from top of paper (1 below top margin) The style and location of the About the Author header matches those of all other major headers (Options include: All capitals or upper and lower case; centered or left aligned) No page number is included on this page The About the Author listing in the table of Contents uses the term End Page in place of a page number

TABLES CHECKLIST Font is the tables is the same as the remainder of the manuscript (font may be reduced to as low as 8 pt but must be eligible) All Tables are numbered consecutively and typed in Arabic numerals using a consistent style The title is placed ABOVE all tables Table notes are placed below the table All Tables are inserted within the manuscript in a consistent manner (within the text, on a free standing page or compiled at the end of each chapter) First sentence of table title matches verbatim to the text listed in the Table of Contents The page numbers associated with each figure matches to the page number listed in the Table of Contents
