Performance Guidelines Please read carefully and INITIAL each section. Approval Process All groups must be approved by Universal Studios Hollywood for performance. Please provide a DVD, VHS or weblink (YouTube, etc) showing an example of the group s performance within the last year, as well as a photograph clearly showing the type of costume to be worn. While the performance submitted doesn t have to be the exact show to be performed at Universal, all music must be pre-approved by a Universal representative before the performance at Universal Studios Hollywood. Materials will not be returned. Universal Studios Hollywood encourages a fast-paced, light program of popular music. Hollywood theme scores are particularly desirable. All selections must be secular in nature. We respectfully request that all music performances not incorporate music associated with Disney All performance application materials must be received at least four (4) weeks and no more than one (1) year prior to the proposed performance date. Incomplete applications will not be approved. Please provide four (4) performance dates in order of priority. Select dates that are free of possible conflicts such as competitions, group arrival or group departure. (Universal will accommodate the 1st choice date when possible; however schedules may present a conflict, requiring additional dates to be listed.) Approval Criteria Acceptance to perform in the Lights, Camera, Music! program is based on the performance quality and stage presence of the group, as judged by Universal Studios Hollywood s Entertainment Department in its sole discretion. Performance Change or Cancellation Performance changes and cancellations require two (2) weeks notification. All performances are subject to change or cancellation due to production conflicts, studio operations, or inclement weather. Best efforts will be made to inform performing groups of any changes in itinerary well in advance of the performance date. Policies and Procedures The Director/Teacher is responsible for distributing to students/performers all pertinent information to ensure that all performance groups are versed in the policies and procedures of Universal Studios Hollywood. Any failure to comply with the policies and procedures at Universal Studios Hollywood shall be grounds for immediate termination of a group s performance at Universal Studios Hollywood. Performance Groups Performance groups are defined as choirs, dance teams, drill teams and cheer squads. Unfortunately Universal Studios Hollywood is unable to accommodate instrumental performances at this time. Performance Requirements One (1) chaperone per ten (10) performing students. Minimum 30 paid tickets per group including non-performers. Coordinated performance attire required. Group must bring own instruments to Universal Studios Hollywood. Groups requesting additional equipment, instruments or making special requests, must do so in advance, and are subject to additional fees. Sound Sound: Maximum 85 decibels at 50 paces. Group shall at all times conform with Universal s direction as to sound. Setup/Performance/Strike Times I understand that my group will have 30 minutes to perform. I understand that an entertainment representative will be contacting me to go over set up and strike times. 3

Performance Guidelines (Continued) Please read carefully and INITIAL each section. Pre-payment Policy Groups performing in the Lights, Camera, Music! program are required to pre-purchase tickets a minimum of 30 working days prior to arrival or performances will be cancelled. Groups are solely responsible for all costs associated with performing at Universal Studios Hollywood, including but not limited to transportation and lodging costs. No compensation will be paid to groups in connection with their performances. Acceptable forms of payment: School/Organization Check or Credit Card. Unacceptable forms of payment: Personal checks, unauthorized P.O. s or payment vouchers. Arrival Universal Studios Hollywood is located in Universal City, California just minutes from Hollywood. Take the 101 Freeway (North or South) and exit at Lankershim Blvd. Follow signs to the Bus Passenger Drop-Off area (map is attached). A Universal Studios Hollywood representative will be on hand to meet your group. Performing groups must wait at the Bus Passenger Drop-Off area until your Universal representative arrives. The representative meeting your group will escort you and your performers to the stage. Do not depart from the Bus Passenger Drop-off area until your Universal Representative arrives. Wardrobe Weather appropriate attire is recommended. School uniforms or hot-weather attire are appropriate performance wardrobe. Matching shirts (with collars) and matching pants are permitted as a substitute for heavy winter uniforms. Jeans and T-shirts are not considered appropriate performance attire. Uniforms/costumes should be suitable for the season performing. Shorts are permitted ONLY by pre-approval from a Universal Studios Hollywood representative and ONLY if matching socks and footwear are worn by EACH MEMBER of the performing group. Universal Studios Hollywood reserves the right to cancel any group s performance at any time if said group s uniforms/costumes do not meet the guidelines of the Lights, Camera, Music! Program. Performers must be in full wardrobe upon arrival to the park. Changing Facility Performance groups are to change prior to their arrival to Universal Studios Hollywood. CHANGING FACILITIES ARE NOT AVAILABLE AT. Lockers are available in the park for paid use. Instrument Storage Performance groups will store instruments on their bus. STORAGE FACILITIES AND INSTRUMENTS ARE NOT AVAILABLE AT. Groups are responsible for providing their own instruments. First Aid GROUPS WITHOUT CONSENT FOR MEDICAL TREATMENT FORMS WILL BE DENIED PERFORMANCE. If a performer becomes ill or has an injury, a chaperone will be asked to either stay with the student until he/she is released by Universal Studios Hollywood First Aid or to check-in with First Aid every 30 minutes. 4

Performance Guidelines & Conduct All members and chaperones of performing groups must abide by the following rules in order to perform at Universal Studios Hollywood. Stage Rules 1. Performance group shall not utilize any sound amplification equipment without prior written approval thereof, and not operate any such approved amplification equipment above the sound levels expressly approved by Universal, and shall at all times conform with Universal s direction as to sound. In the event of noncompliance by Performance Group, Universal reserves the right to shut down the Event until the sound equipment is adjusted. Universal shall, in their sole judgment, expressly set sound levels; in no event shall any sound level exceed 85 decibels at 50 paces. 2. No obscene language or verbal abuse of any sort. No use of language intended to and/or having the effect of inciting the audiences to abusive behavior. No derogatory statements about Universal Studios, Universal CityWalk, the tenants on property, or any partners associated with NBCUniversal. 3. No use of alcohol, drugs, or smoking prior to or during stage time, or any time while on Universal s premises. 4. No literature distribution, no stickers or other promotional items without prior consent from Universal Studios. 5. No obscene or immoral conduct. No violence. 6. No pyrotechnics, fire, candles or any sort of open flame. 7 All music must be secular in nature. 8. No acts of worship, seances, hallucinations or convulsions. 9. No knives, guns, sticks or weapon-like objects on Universal property. 10. No damaging stage, microphones, amps, lights, drums, cables or any other property belonging to Universal City. All performance groups will be held liable for current replacement value of damaged objects. 11. Must remain professional at all times and must remain in full uniform/costume until performance is complete and group is out of guest view. 12. NO PHOTOS OR VIDEOTAPING PERMITTED IN BACKSTAGE AREAS. Employees of Universal Studios retain the right to disqualify, refuse stage time, cancel, or cease group performance if any performance group member violates any one of the above rules, as determined by Universal Studios in its sole discretion. I agree to the above rules on behalf of my performance group and acknowledge that any member or entire performance group will forfeit our performance if any one of these rules are violated by myself or any one performance member. 5 Towers Stage The 5 Towers Stage is 28 wide by 21 deep. The deck is 39 high. The stage is outdoors, uncovered and subject to the elements. 8

Waiver of Liability and Assumption of Risk & Indemnity Agreement In consideration of being permitted to officiate, observe, or participate in any way in the Lights, Camera, Music! Program (the Event ) or being permitted to enter for any purpose any restricted area (defined as any area requiring special authorization, credentials, or permission to enter or any area to which admission by the general public is restricted or prohibited), each of the undersigned, for himself, his personal representative, heirs and next of kin: 1. Hereby grants Universal City Studios LLC dba Universal Studios Hollywood and its Affiliates and their successors, assigns, and licensees, permission to photograph, tape and/or film me, record my voice and likeness for advertising, publicity and promotional activities related to the Event, to Universal Studios Hollywood generally, and/or for archival purposes only; 2. Hereby grants this permission to you with knowledge that you will be relying on and expending substantial sums based on this grant of permission, and I accordingly agree not to assert any claim against you of any nature whatsoever arising by reason of your use of such elements; 3. Hereby releases, waives, discharges and covenants not to sue Universal City Studios LLC its parent company/affiliates, and its and their respective officers, agents, directors, employees, licensees and invitees (collectively Universal ), for any and all loss or damage, and any claim or demands therefore on account of injury arising out of or related to my participation in the Event, whether caused by the negligence of Universal or otherwise; 4. Hereby assumes full responsibility for any risk or bodily injury, death or property damage arising out of or related to the Event whether caused by the negligence of Universal or otherwise; 5. Hereby agrees that this Publicity Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement extends to all acts of negligence by the releasees, and is intended to be as broad and inclusive as is permitted by the laws of California and that if any portion thereof is held invalid it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. I have read this Publicity Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement and fully understand its terms, understand that I have given up substantial rights by signing it, and have signed it freely and voluntarily without inducement, assurance, or guarantee being made to me and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law. Directions to Bus Loading From the South Bay: Take the 105 Freeway East to the 110 Harbor Freeway North. After passing through the downtown area, get on the 101 Hollywood Freeway North. Follow the 101 to the Universal Studios Blvd. exit. Turn right onto Universal Studios Blvd. PLEASE SEE BELOW From Pasadena/Burbank/Glendale: Take the 134 Freeway West and exit at Cahuenga Blvd. Turn left and head South on Cahuenga, Cahuenga will merge onto Lankershim Blvd. Veer left onto Lankershim proceed toward the freeway (past Universal Bldg on your left). Go under the 101 Freeway and turn left at the light onto Cahuenga Blvd. (aka Ventura/Highland). Continue on Cahuenga and take a left onto Universal Studios Blvd. at the light. You will pass back over the 101 Freeway and proceed up the hill. PLEASE SEE BELOW After exiting onto Universal Studios Blvd., buses should follow the signs for "BUSES/DELIVERIES." At the parking tollbooth, the attendants can direct you to the Bus Parking Area. Buses can remain in this parking lot for the duration of the performance ONLY. At the conclusion of the performance, buses will need to move to FREE bus parking on Buddy Holly Dr. located south of CityWalk. If buses wish to remain parked in the bus parking lot for the entire day, you will need to alert the tollbooth upon entrance and pay a bus parking fee.
