1st Revised Sheet 1 Cancels Original Sheet 1 SERVICE CHARGES Service Charges are Nonrecurring charges applicable for the following: Service Connections - Establishment of telephone service for a customer or subsequent additions to that service. No distinction is made between a "New Installation" and an "Outside Move." Inside Moves - Transfer of existing service or equipment from one location to another on the same premises and in the same building. Equipment and Wiring Changes - Substitutions of a different style or type of equipment, or a rearrangement of equipment or wiring which does not involve a change in location of equipment. Records Work Only - Customer requested changes or additions affecting records only. Service Charges are in addition to all other rates and charges for service and equipment. They apply in addition to and not in lieu of any applicable Initial, Nonrecurring, Installation and Other Charges specified by this Guidebook or Construction Charges made because of unusual cost or conditions in establishing, moving, or changing service. (T) Service Charges apply for the following operations required to establish, add to, move or change telephone equipment and service: Service Request Charges - Receiving, recording and processing of a customer's request. Central Office Line Connection Charges - Performing the work operations associated with the line extending from the serving central office to the customer's premises including, but not limited to, central office connections, cable cross connections, and connecting the drop wire and protector. Service Charges also include Records Work Only Charges -- the receiving, recording, processing, and taking action in connection with customer requested changes or additions to records only including, but not limited to, directory listings and Joint User Service.

1st Revised Sheet 2 Cancels Original Sheet 2 SERVICE CHARGES (cont'd) Service Charges will not apply to the following: - Installation, moves or changes of public telephone service. - In the case of damage to or destruction of the customer's premise due to fire, flood, or other catastrophe, the same service will be reestablished without charge, either at the same or a different location. However, Service Charges will apply to any temporary service installed pending reestablishment of the customer's regular service, except as provided pursuant to the Major Disaster Relief Plan as shown in Part 2, Section 2, of this Guidebook. (T) - Moves or changes required for the proper maintenance of the equipment or service or made at the initiative of the Telephone Company for service reasons. - Exhibition Hall Service. - To the restoral of service which has been on Temporary Suspension of Service. - Upgrades from party line service to flat rate single party service. - For Company-sponsored promotional offerings see Paragraph 3.6. - To initially request blocking of calls to 900 and/or 976 Information Services numbers. - To change 900 and/or 976 blocking if initiated within 90 days of when 976 service is available in the customer's area. - Subsequent additions or changes if initiated within 90 days of a residence Initial Service Connection. - Moves between the options offered within Line-Backer Maintenance agreements. - Initial Service Connection and Line Connection charges do not apply for Business customers who have their local service with another competitive carrier and wish to establish exchange access lines or trunks with the Company. Customers must commit to a minimum 1-year line or trunk term commitment (e.g., CompleteLink) and must not have had service disconnected for non-payment nor have any past due bills for regulated service owed to the Company in order to qualify. This may not be combined with other access line nonrecurring charge waiver offers at the same time and is not applicable to Flexline, Centrex, ISDN or coin service lines nor to additional lines ordered after the initial installation order.

1st Revised Sheet 4 Cancels Original Sheet 4 SERVICE CHARGES (cont'd) Either one or a combination of the Service Charges will apply depending on the operations involved to meet the customer's request. Except where specific charges are quoted in this Guidebook, all other equipment, facilities, and wiring will be installed, moved, or changed at cost. (T) Service Request Charges Service Request Charges are divided into two categories: Initial Service Connection - for connecting new or additional central office lines. Subsequent - for moving or changing existing service and equipment or adding new or additional service and equipment other than central office lines. One Service Request Charge (either Initial or Subsequent) applies for each request made for telephone equipment and services to be completed for the same account, at the same premises, at the same time. Unless otherwise specified, a Service Request Charge - Subsequent applies in addition to any other Nonrecurring charges which may be applicable for telephone equipment or service furnished following the initial service connection. A Service Request Charge - Initial is applicable when a telephone number change is made at the customer's request and is not required for the proper operation of the service. This charge is not applicable when telephone number changes are initiated by the Telephone Company. A Service Request Charge - Initial is applicable when a change in class or grade of service is made at the customer's request and is not required for the proper operation of the service. This charge is not applicable when changes in grade of service are initiated by the Telephone Company or when the change in class or grade of service is from a party line to a flat rate single party

1st Revised Sheet 5 Cancels Original Sheet 5 SERVICE CHARGES (cont'd) A Service Request Charge - Subsequent is applicable for a change in name and responsibility except if there is no interruption in the residential service and the service is transferred to a subscriber who resides in the same household at the time of the transfer or to a subscriber who will reside with the original customer in which case a Records Work Only Charge applies. A Service Request Charge - Initial is applicable for changing lines from or to Rotary Hunting Service. A Service Request Charge - Subsequent is applicable to subsequent requests for blocking of calls to 900 and 976 Information Services numbers. Central Office Line Connection Charge A Central Office Line Connection Charge is applicable for connecting an access line from each serving central office; it does not apply to the transfer of an existing central office line serving a reseller s end user when there is no interruption of service. Central Office Line Connection Charges are not applicable to CENTREX service. Records Work Only Charge The Records Work Only Charge is applicable for customer initiated requests for changes and/or additions to records only. The charge is waived if any other Service Charge is applicable to the customer s request. A Records Work Only Charge is applicable for customer requested changes in their listed name or address, or changes in method of billing for services. A Records Work Only Charge is not applicable to establish or change the customer billing name and/or address, change in responsibility without a change in the listed name, or requests for Ameritech Calling Cards.

1st Revised Sheet 6 Cancels Original Sheet 6 SERVICE CHARGES (cont'd) Restoral of Service Charge Where the service of a customer has been suspended due to violation of the Telephone Company's rules and regulations, but the service has not been terminated or the order to remove the service has not been completed, such service will be restored upon the payment of an amount equal to the appropriate Service Request Charge for Initial Service Connection as shown in this Part. In case service has been suspended for non-payment of charges due, in addition to the restoration of service charge, the customer will be required to make satisfactory arrangements as to the payment of out-standing charges, and may also be required, if considered necessary by the Telephone Company, to make a cash deposit to provide satisfactory security in the future. Subsequent to the completion of an order to discontinue service, the service will be reestablished only upon the basis of a new service application, subject to the provisions contained in Part 2 (Advance Payments and Deposits), and subject to the usual Service Charges to connect service.

1st Revised Sheet 7 Cancels Original Sheet 7 SERVICE CHARGES (cont d) Premises Work Charges Premises Work Charges apply for customer requested installations, moves or changes of this Company's network facility on the customer's premises, excluding work subject to Rearrangement Charges provided in this section. Premises Work Charges are based on the time spent at the customer's premises performing actual work, including work preparation and clean up. When more than one craftsperson is involved in the premises work, charges are based on the total time for each person. The initial 15 minute increment applies to the first 15 minutes of work on the customer's premises. The remaining premises time is charged as additional increments, even if the requested work requires more than one visit for Telephone Company reasons. At the customer's request, a firm bid will be quoted in advance for work which is subject to Premises Work Charges. This quotation will be valid for a stated period of time. If the bid is accepted by the customer and the work is performed, the amount of the bid applies in lieu of the stated charges. Title to any wire, cable, or materials used remains with the Telephone Company. Rearrangement Charges Performing customer requested moves or changes to the drop wire and/or protector, and moves, changes and additions of a Standard Network Interface (SNI) including any facility between the SNI and the protector.

1st Revised Sheet 11 Cancels Original Sheet 11 JACKS Standard Voice Jacks and Standard Data Jacks The right is reserved to restrict the jacks installed in connection with any one line to such a number as, in the judgment of the Company, will not adversely affect the service. Nonrecurring Charges are applicable only when jacks are installed as the termination of an access line or private line channel. Non-Standard Voice Jacks Non-Standard Voice Jacks are not to be used for the connection of FCC registered terminal equipment or systems. Move and Change Charges Same as for an initial installation. Extension Service Channel Charges Extension service channel charges, as provided in Part 4 are applicable. Nonrecurring charge does not apply for jacks when installed or changed at the initiative of the Telephone Company or to in-place jack equipment. Apartment Door Answering System Jacks Provides a maximum of twelve line terminations /RJ71M/ (The customer/customer's vendor must furnish the Telephone Company with a list of the existing telephone numbers in the apartment building.) Maintenance of Service Charge is not applicable when a service difficulty is due to trouble located in the Apartment Door Answering System. A maximum of two hours of Premises Work Charges, specified in this Section, are applicable. When in this Company's judgment the customer's requirements are such as to necessitate more than two hours of Premises Work Charges, the Telephone Company will furnish the customer with a written estimate.

1st Revised Sheet 13 Cancels Original Sheet 13 INSTITUTIONAL PROGRAM FOR PREMISES WIRING CHARGE The Institutional Program for Premises Wiring Charge provided in the Ameritech Tariff, Part 2 is comprised of the first, or a combination of both rate elements described below, as appropriate: - Element 1 - for the first fifteen minutes or fraction thereof, of total time spent by Telephone Company employees in related monitoring or participation in acceptance tests and/or in related inspection of customer-provided premises wiring for Communications Systems subject to Part 68 of the Federal Communications Commission's Rules and Regulations, and for administrative expenses including the visit to the customer's premises. - Element 2 - for each additional fifteen minutes, or major fraction thereof, of total time spent by Telephone Company employees in performing the related customer-premises activities described in element 1 preceding.

1st Revised Sheet 15 Cancels Original Sheet 15 MAINTENANCE OF SERVICE CHARGE A Nonrecurring Charge applies for each repair visit to a customer s premises in connection with a service difficulty when it is determined that the difficulty was due to a condition in a customer-provided terminal equipment, communications systems or accessory associated with the service. Repair visits due to trouble in customer premises inside wire are also subject to this charge, as provided in Part 2, Section 2 of this Guidebook. (T) Customers that subscribe to one of the Line-Backer maintenance Agreements will not be subject to this charge. For purposes of applying maintenance of Service Charges, Single-Line and Multi-Line Business are defined as follows: Single-Line Business - one or more lines terminated in individual line (non-key) terminating equipment (i.e., telephone sets, recording devices, etc.). Multi-Line Business - one or more lines terminated in, but not limited to, key telephone systems, PBX switches, Centrex service, multi-function systems, and Data arrangements.
