Attached are contracts/release forms for the 2017-18 school year. These forms need to be read, signed and returned by Friday, Sept. 1, 2017. Failure to turn in forms by 9/1/17 will not only affect your grade, but your ability to perform and/or remain in Concert Choir. You should have already read the SHS Choral Constitution which was emailed through Skyward and available for download on the school website. The required student fee for Concert is listed below. The payment due dates are as follows: Friday, September 9 th, 2017 $ 40 Friday, November 17 th, 2017 $ 30 Concert T-shirt Yearbook Banquet Music Accompanist Supplies Total Chorus Dues $7 $2.00 $26 $20 $10 $5 $70 Student accounts must be paid in full by Friday, Nov. 17 th, 2017. Students whose accounts are not up-to-date will not be allowed to perform at Disney Candlelight. The Mixed Bag Designs Fundraiser starts Oct. 10 th and goes until Oct. 27 th. Money raised through Mixed Bags (40%) goes directly into student accounts and can replace direct payments. Students are encouraged to participate in chorus fundraisers to pay off their accounts. Students in financial need must communicate to Ms. Parkhurst via parent email and scholarships will be awarded on a case-by-case basis. We do not have a choral assistant this year and will be looking for dividends to assist more than ever! Please be sure to register on the SCPS website. You must re-register every year and it takes 3 weeks to clear. You will receive an email once you have been approved. Please indicate your intent to help on the parent volunteer form Included in this packet. Destiny will be performing in the Pops and Pastries Performance and Parent Meeting on Oct. 5 th at 7:00p. All parents are encouraged to attend. We are accepting pre-package donations of pastries, sweets, canned soda, and water bottles for the event. Feel free to contact me with any questions or concerns. 2 SHS Choral Contract Concert Choir Mandatory Performances/Rehearsal Schedule Thursday, December 14 2:30-5:00p Winter Chorus Concert Rehearsal Friday, December 15 5:45p Call Winter Chorus Concert @ 7p Saturday, May 5 9:00a-9:30p Spring Concert/Rehearsal-Call times TBD PERFORMANCE ATTIRE Maintaining a professional appearance is a part of your grade. Students failing to meet the uniform requirements will not be allowed to perform. Destiny costumes and uniforms are purchased by the program and students pay a portion of their dues for costume upkeep, cleaning, repairs/alterations, etc. There are also additional items (mentioned in the audition contract) that students are required to purchase individually and use throughout the school year. In many cases, students can purchase these items directly through the program. Students are expected to wash and maintain these items personally. I ve included a bulleted list of these items below. All members need: Black Chorus t-shirt (Provided by SHS Choral Department) Long, Solid black dress pants (No capri, jeans, leggings/yoga pants, sweat pants or skinny pants) Plain Black Socks or stockings Solid Black, Closed Toe/Back, Dress Shoes (no tennis shoes, combat boots, or flip flops, etc.) Additional performance attire requirements: HAIR: All performers are required to have natural hair color for any of their performances/auditions. For performances, hair color/styles must not be distracting and must not be in the face of the performer. During rehearsal all hair should be pulled back into a pony tail. JEWELRY: For Disney NO JEWELRY may be worn at any performance. Jewelry will be assigned to each performer and must be return after the performance. Students should not wear any personal jewelry during rehearsal/performances. FINGERNAIL POLISH: For all performances, fingernails must be natural or nude/clear. Bright colors, long nails, or distracting manicures are not allowed. French manicures are acceptable. Students wishing to audition for Disney s Candlelight Processional have a separate dress code, which includes: White Chorus t-shirt - $7.00 purchased through SHS Choral Department Long, Solid black dress pants (No capri, jeans, leggings/yoga pants, sweat pants or skinny pants) Plain Black Socks or stockings Solid Black, Closed Toe/Back shoes (no tennis shoes, combat boots, or flip flops, sandals, etc.) You will not be allowed to attend if you fail to meet the dress code requirement

3 SHS Choral Contract Concert Choir AGREEMENT I,, have read the 2017-2018 Choral Constitution and Concert Choir Contract and understand all that is required of me. It is my responsibility to advise my parent/guardian of the information put forward by the SHS Choral Department and return all paperwork by the assigned deadline. I solemnly promise to follow the rules and regulations as set forth in the constitution at all times during the upcoming academic year. I furthermore understand that I am to maintain a minimum 2.0 Grade Point Average per nine weeks, adhere stringently to SHS attendance and behavior policies and abide by the No Tolerance regulations as set forth by the School Board of Seminole County. I recognize that I am a role model for other Seminole High School students and agree to conduct myself, at all times, in a manner that will make my parents, teachers, administrators and community creditable and proud. Signed (Student) I/We have read the Choral Department Constitution and Concert Choir Contract and understand all the requirements therein. I/We am/are the parent(s) or legal guardian(s) of the above named choral member and by signing this contract, I/We am/are giving my/our permission for my child to participate in the above named choral ensemble for the 2017-2018 school year. In addition, my/our child has my/our approval to participate in all fund-raisers understanding that, if my/our child s account is not paid, or if funds raised are not turned in, I/we will be held responsible. It is my/our responsibility to have my/our child communicate with me/us on the availability of fundraisers, planned trips and competition(s). I/We understand that if my/our child is injured or becomes sick, the school board of Seminole County, Florida will not be liable unless the injury or illness is the result of negligent conduct of the part of an employee of the School Board of Seminole County, Florida. 5 SHS Choral Contract SHS Choral Dividends Do you want to be involved? Come be a part of the choral program as we work together to assist the SHS Chorus. Choral Dividends serve a very important role. Specifically, parents work in one of the areas selected (see below).
