Dynamic Tables What are Dynamic Tables? Used to collect data in the form Located in the following sections of FCC Form 481: 200 (outages) 700 (voice pricing) 710 (broadband pricing), and 800 (operating companies) Data can be populated before even accessing the E-File system through use of Excel templates which are available on USAC s website (Forms page). Using the Templates Ensure that all fields are completed in the Excel template before uploading. To assure successful template upload: Carriers cannot modify the columns. The first row must remain as column headers. Dynamic Tables Data should be entered on the second row and each row thereafter. Each row of data must be in the proper sequence matching the column titles and compliant with formats outlined in the FCC Form 481 instructions. High Cost Program FCC Form 481 9

Dynamic Tables Using the Templates Two versions available Excel 2007 and Excel 2010 Ensure the correct version is used Do not attempt to use templates from a prior year filing as the fields may have changed. (100) Service Quality Improvement Five Year Plans & Progress Reports Price cap carriers are not required to file an initial five-year plan this year Rate-of-Return carriers who filed an initial five-year plan last year will now be required to submit a progress report (attached to Line 112) Carriers are not required to submit a new five-year plan if they previously filed a five-year plan Progress reports should address the current calendar year (2015) up to the FCC Form 481 filing date Progress reports need to be in compliance with 47 C.F.R. 54.313(a)(1) High Cost Program FCC Form 481 23

(100) Service Quality Improvement Five Year Plans & Progress Reports Per the C.F.R. - A progress report on its five-year service quality improvement plan pursuant to 54.202(a), including maps detailing its progress towards meeting its plan targets (Line 113), an explanation of how much universal service support was received (Line 114), how (and how much) support was used to improve service quality (Line 115), improve coverage (Line 116), or improve capacity (Line 117), and an explanation regarding any network improvement targets that have not been fulfilled in the prior calendar year (Line 118). (1100) No Terrestrial Backhaul Terrestrial backhaul refers to a land-based connection to the broadband backbone network. On the Data Collection Form, if a carrier selects No, that it does not have terrestrial backhaul options, then it must complete the No Terrestrial Backhaul section. High Cost Program FCC Form 481 38

(1100) No Terrestrial Backhaul Lines 1120 and 1130 have drop-down boxes. Line 1120 addresses whether terrestrial backhaul options exist within the supported area. Line 2010 Recipients of Incremental Connect America Phase I support should respond whether they have deployed broadband service to no fewer than two-thirds of the required locations ( 54.313(b)(1)(i)). (2005) Price Cap Data Possible Certification Requirement for Price Cap Carriers Line 2014 Recipients of Frozen support should respond whether two-thirds of the support received in 2014 was used to build and operate broadband-capable networks to offer broadband in areas substantially unserved by an unsubsidized competitor ( 54.313(c)(3)). The ETC must certify that at least 50% of the locations served are in census blocks shown as unserved on the National Broadband Map (FCC 11-161, paragraph 149, footnote 238). (2005) Price Cap Data Possible Certification Requirement for Price Cap Carriers Line 2016 (For recipients of Connect America Fund ICC support) Any ETC receiving High Cost Program support to offset access charges pursuant to 47 C.F.R. 54.304 should respond whether all of the support received was used to build and operate broadband-capable networks to offer broadband in areas substantially unserved by an unsubsidized competitor ( 54.313(d)). The ETC must certify that at least 50% of the locations served are in census blocks shown as unserved on the National Broadband Map (FCC 11-161, paragraph 17995, footnote 1820). (3005) Rate of Return Data Rate of Return carriers are required to address new requirements for the 2016 program year: Line 3010 Carriers must certify they have taken reasonable steps to provide upon reasonable request broadband service at actual speeds of 4 Mbps downstream/1 Mbps upstream, with latency suitable for real-time applications, including Voice over Internet Protocol, and usage capacity that is reasonably comparable to reasonably comparable offerings in urban areas, and that requests for such service are met within a reasonable amount of time ( 54.313(f)(1)(i)). Note that although the FCC recently adopted 10/1 speeds, the correct speeds for this certification are 4/1 Mbps. Line 3011 Carriers must confirm that the attachment on Line 3012 contains the required information pursuant to 54.313(f)(1)(ii) including the number, names, and addresses of community anchor institutions to which the ETC began offering broadband service in the preceding calendar year. Line 3012 Attachment of community anchor institution data referenced in Line 3011 ( 54.313(f)(1)(ii)). High Cost Program FCC Form 481 51
