3 FORMAT REQUIREMENTS/GUIDELINES Spacing Double space the general text of the manuscript. Single-space within each bibliographical entry and double-space between entries. Single-space within long tables, long quotations, footnotes, appendices and multiline captions. Justification Left justification (not full) for all text, including footnotes and bibliography, unless otherwise specified in the Formatting Guide. Appendices are not bound by this requirement Margins First Page of Introduction or Chapter: Top: 2 (50.8 mm) Right, and Bottom 1 (25.4 mm) Left 1.5 (38 mm) All other pages: Top, Bottom and Right: 1 (25.4 mm) Left 1.5 (38 mm) Fonts Times New Roman (or equivalent), 12-point fonts for text and 10-point font for footnotes and subscripts. Footnotes Footnotes numbers run sequentially from the first to the last page of text. Footnote numbers are superscripted before the first character of the footnote text. Otherwise, footnotes conform to Kate Turabian s Manual for Writers, 8 th edition (see Appendix A for examples). Citations within the text are limited to Biblical references (as well as other sacred documents). Footnotes conform to the margins of the page. Footnote text must be in 10 point Times New Roman, with the exception or the footnote number that can be no smaller than 8-point font size. Footnotes (along with the footnote number) have a first line indent of.5 (12.7 mm.) from the left margin. Footnotes are single spaced within, and double spaced between. Printing and Copying No cross-outs, lines, smudges and smears, and white-out. Print single sided unless professor states otherwise. Paper White paper, clean copy paper A1
4 PAGE NUMBERING AND PLACEMENT (See Table 1 on next page) Preliminary Pages Preliminary pages refers to all pages that precede the text of the paper Count, do not number, the title page, Copyright page (if included), and Abstract. Count and number with roman numerals (iii, iv, v, etc.) the acknowledgement page, and the table of contents. Numbers are placed in the center of the page 0.5" (12.7 mm) from the bottom. Text, Appendices and Bibliography Count and consecutively number with an Arabic number (1, 2, 3) all text pages, appendixes and bibliography. Appendixes continue numbering from the text of the paper. Scanned documents that are already numbered must be reduced so they display the page numbering of the document. Page numbers are placed in the footer of each page, centered 0.5" (12.7 mm) from the bottom. At the top of the bibliography should be the single word Bibliography followed by a two double spaces. A2
5 Table- Pagination and Sequencing The following table shows the proper sequence for pagination. These areas may or not be applicable to your paper. Required for most academic papers are: title page, body of paper, and bibliography. Your professor will specify which elements that your paper must have. 6 Examples of Formatting Guidelines Cover Page Do not add cover page to the total number pages Do not include graphics Centered in with 2 top margin type the TITLE in CAPS Double Space before and after By Space down 3 and type the following in Title Case: Author, Course Number and Name (Optional Instructor) Date First Page of Text Top margin should be set at 2". Type INTRODUCTION" if you are beginning with an introduction or name of chapter or section. Double-space twice. Begin typing the doublespaced text, indented at the beginning of each paragraph. Do not put extra spacing between paragraphs. Begin re-counting page numbering at 1 in Arabic numerals (1, 2, 3, etc.) at the bottom of the page. The first page of all subsequent chapters will take this format and begin with a top margin = 2". Subsequent pages will have a top margin=1". A4
7 Subsequent Pages of Text Top margin should be set at 1". Continue counting numbers. Appendix APPENDIX A centered 1 (25.4 mm) from the top of the page. Double-space twice and type the title. Double space and begin text or input graphic. A5
8 Bibliography Entries Type the heading Bibliography centered 1 (25.4 mm) from the top of the page. Double-space twice. Type the list of sources. The list of sources is single-spaced within, and double-spaced between, entries. Entries should be formatted with a hanging indent. (The first line is flush left and subsequent lines within the entry are indented.5 inches from the left margin.) A6

21 A new section/chapter once again has a 2 top margin. 2 Top Margin Review of Literature pages: Review of literature contains is a general overview of the academic writing that is related to your area of research. Like all major sections, the top margin is 2.The review of literature is more than a summary of all the information that exists in relation to your topic, but a synthesis of that information in way that gives an understanding of the current state of the topic being researched. This means that the review is a historical/social/geographic interrelationship of scholarly research. Include ten to twenty books and/or articles. Narrow down and reformulate your research question(s), based on your literature review. NYTS Formatting style requires that footnote numbers run consecutively through document, rather than restarting at the beginning of each chapter or section. 3 Even though this footnote is in the second section, the number follows from the previous one. 3
22 2 Top Margin Context of the Project 4-5 pages: All research exists within the context of the subject being researched and the context of the researcher. Contexts can be result from different space or different time. Context includes the subjects of research as well as that of the researcher. The institutional context also includes other relevant research going on in the institution. 4 In research it is essential that they be well defined in order to be able to produce reproducible data and conclusions. The project exists in the context of your desire for a grade and diploma, the contexts and desires of the subjects who are part of the project and the political historical space in which the project is performed. 5 Your research setting: Church, community, organization,etc. 4 Lisl Klein, Working Across the Gap: The Practice of Social Science in Organizations. 23 2 Top Margin Theological Perspective 5 pages: One of the major contexts and lenses that of Pastoral Care is theological. What differentiates Pastoral Care and Religious Education from other forms of counseling is that the theological question is considered essential. We begin with the church and add to the understanding of care, the developments of areas such as psychology or sociology. But the theological understanding is paramount to these disciplines. Since you will be looking at the Bible or other sacred documents as primary, they will be cited but in a different way than other authorities. The Bible is the only authority that is cited in the body of the text rather in a footnote. Rather than a footnote, a citation is in parentheses followed by the abbreviation for the translation. For instance, the introduction of the Gospel of a John would be cited as: In the beginning was the Word (John 1:1 NRSV). NRSV is the abbreviation for New Revised Standard edition. Since this is a direct quote it is surrounded by double quotes like any direct quote, but it is cited within the text rather than at the bottom of the page. 5
24 2 Top Margin Psychological Perspective 5 pages: It is essential to be able to see actions and relations outside of theological structures as well as part of them. Otherwise, the pastoral counselor cannot understand behavior outside of the church or religious framework. The psychological perspective provides a lens to observe and understand behavior, that when combined with the theological perspective provides a more holistic understanding of the individual. Observation is at the basis of psychology. 6
25 2 Top Margin Social and Cultural Analysis 5 pages: Formation is the result of the interaction between biology and society. We are not only our genes, but our environment as well. Therefore, an analysis of the social and cultural dimensions of an individual s life is essential. Social and Cultural Analysis makes up the third leg of the supporting structure of Pastoral Counseling: Theology, Psychology, and Social and Cultural Analysis. 7

27 1.0 Margin 1.5 Left Margin Introduction (1 page) Verbatim (3-4 pages) See Following Example Nancy Smith Date of visit: 11/18/2015 Length of visit: 30 minutes (1:15-1:45 p.m.) 1.0 Right Margin Before the visit: This patient is new to me but not new to hospice. I am taking over a few patients from one of our chaplains, and it was recommended that I see this patient as she has expressed interest in spiritual support and seems to gain a lot from it. I knew very little about her, but knew that she was Catholic, enjoyed hymns and scripture, and seems lonely. She was admitted to hospice under decline in clinical status, which a very vague term indicating that the person s health is failing for no specific reason other than general decline and age. The case manager had recently mentioned to me that this patient might benefit from twice-monthly visits, which is more than our usual. I planned on going in to introduce myself and develop some rapport with her, see about her needs and wants, and then plan what next steps would be. I had just come from a long visit with another new patient, and I was feeling tired as well as frustrated and angry because this prior patient s dog bit me twice on my leg as I was leaving his home. I was practicing uptime during the day by leaving my car radio off while traveling and being more accepting of the relative silence. This helped keep me focused on what I was doing and not distracted. 9
28 During the visit: Mrs. S was in bed, the room was dim. Staff escorted to me to her room and said that they had just laid her down as she said that she was tired. They noted nothing else out of the ordinary. Her TV was off, there were a couple bulletin boards in her room with pictures of family and a couple cards. She did not have a roommate present at the time. When the nurse awoke her to introduce me she seemed scared and anxious, not knowing who I was or what was going on. She did not have dentures and spoke softly, which made her hard to understand at times. Visit narrative C: Chaplain P: Patient C1 Hello, Mrs. S? P1 Yes? C2 Hi, I m Sam Blair. I m a minister with Gateway, the extra nurses that come by to see you. Pastor Wayne who sees you told me that you might like a visit. (sitting down at bedside) P2: Yes. He told me to stay in bed so I don t fall. He said I should stay in bed. (seems a bit anxious) C3 How are you today? P3 Good. I m tired though. C4 Really? P4 Yes. I m 95. C5 Really? P5 Yes. I turned 95 this past weekend. C6 Wow! Well happy birthday! How was it? P6 Well, I thought it would be a big thrill but it wasn t. C7 Well you don t look 95. P7 Everybody says that. But there s nothing really new to say. Nothing new at all. C8 Oh. I m sorry to hear that. (silence) C9 How long have you been here? P9 A couple of months. It feels like always. C10 Do you like it? They have very good jobs. C17 I see. P17 (pauses) I d like to live with her. C18 I see. You re pretty disappointed that you can t. P18 Yes. I ve had a lot of disappointments in my life. I think that some of the most interesting stories are the ones that don t seem interesting to you. To you they re mundane but to other people they re amazing. P31 (smiles) C31 Before I go would you like your TV on? P32 No, I can t watch TV. I used to like to read, but now I can t see well. I had cataracts and I could only have one removed. They had to take me in an ambulance and it was horrible. I went this way and the other way C32 The ride was pretty scary for you. P32 It was. I couldn t go and get my other eye done. I haven t read much Dickens, but I did just read a book by Jack London, The Sea Wolf. London he wrote about Alaska. I remember that the Europeans liked Jack London more and Americans tended to like Dickens. Would you like me to get you some books on tape for you? I like to listen to books on tape when I m driving. No thanks. How about a radio? No, I could ask my daughter, but I couldn t get one I could ask to get one here. No, it would just be another disappointment. (pause) I m tired. I d really like to close my eyes and rest. Ok. Before I go, would you like me to pray with you? (makes eye contact again) Yes. And you re Catholic, right? (looks surprised) Yes. Don t worry. Wayne told me. I didn t just figure it out. Would you like to pray the Lord s Prayer? Yes I would. (We pray the Lord s Prayer together. She has points where she loses track and I try to match her pace). God bless you. I was very glad to meet you. I ll stop by and see you again. I d like that I d like that. Have a good day. After the visit: I thought the visit went well and it ended up being the best one I had this week. There were a few times when I think we both felt a bit uneasy and I tried to wait for her to break the silence rather than interject on my own. I also noted a few times when we both went off track (talk about the weather) but I used these moments to try to join with her as well. I felt her depression and sadness and wanted to reflect an acceptance of herself as she was. I felt sad for her but also felt that the visit had brightened her day a little which made me satisfied and happy. She expressed a desire to continue the relationship which affirmed what I felt as well. The patient was more open with me than I expected at first. I also did not expect her to be so alert and oriented. Most of my 12
31 patients have at least some degree of dementia, and she was quite sharp and insightful. Her comments on Dickens and literature took me quite off guard. Themes of sadness, loneliness, lost dreams and hopelessness came through early on in our conversation. Through most of the conversation she did not make direct eye contact, choosing to either look down or through the corner of her eye. I sensed some sensitivity and afterward noted that I did not use touch as much as I tend to do. She gave me a lot of feedback during the conversation through her body language, as well as through some very direct answers at times her Yes had some passion behind it.
