The Xavier Theatre Academy provides two three-week intensive theatre experiences for serious, impassioned performers and technicians. This is your digital copy of the Academy Audition Registration forms. You may keep this for your records. You do NOT need to print this document or submit printed copies at auditions. This digital copy is for your own records. New for Xavier Theatre Academy this year Both productions will be supervised by Jim Romenesko and Steve Kovacs with stage and musical direction by Megan Dumke and Emily Albrecht, respectively. Parent Form Thank you for your interest in the Xavier Theatre Academy! We re very happy that you are interested in having your son or daughter participate! If you have questions that are not answered by the information in this packet, please email Jim Romenesko ( jromenesko@xaviercatholicschools.org ) Statement of Purpose: The Xavier Theatre Academy is a three-week intensive theatre experience for serious, impassioned performers and technicians. The objective of the Academy is to involve committed students completing grades 1 through 9 in a fully-staged play or musical using professional production practices while educating them in all aspects of theatre production. Student Expectations: Students are expected to participate fully in this endeavor and to allow others to do the same. Actions contrary to this goal are not acceptable and will result in consequences up to and including dismissal from the program with no refund. The Academy Director has the final say in all issues pertaining to the academy operation, casting, and duties/responsibilities during performances. Please be advised that the participation in the academy is based on skills and potential exhibited during the audition as well as availability. This is not a no-cut audition. Please be certain your child understands this before auditioning. Costume Policy: Most costumes are provided by the school. However, the following personal items are required for performers depending on their assigned role. Black ballet or jazz slippers and/or black character shoes (single strap vs. t-strap) for females, black or brown dress shoes (depending on role) for males. More details will be provided to those who are cast in the show(s) once characters are assigned. Undergarments are also the responsibility of the student. Tights, wigs and other costume pieces are provided. Additionally, the hairstyle worn at auditions should be maintained unless you are informed otherwise. Performers should not have drastic changes to hair style (short cuts, coloring, perm, etc) without a consultation with the director. Some modern styles will need to be modified. Expenses (wigs or re-styling made necessary by variance from this rule) are the responsibility of the performer. Tuition: Academy tuition is $315.00 for the JR show and $215.00 for the KIDS show. Students participating in both shows get a discounted price of $115.00 for the KIDS show. Payments will again be accepted through Showtix4u (our ticketing agent) this year with a nominal fee or by check (no fee if paid on time). Full payment details will be included in the acceptance letter sent to students who are cast in the show(s). Tuition includes show t-shirt. Scholarships: There are no scholarships available directly from the theatre academy. However, some employers and school systems do offer scholarship opportunities. The academy will gladly provide documentation to assist you in securing such scholarships. Additional Information: All casting is based on the expectation that the student will be able to learn the material and perform it proficiently, behave in a cooperative, appropriate manner, fulfill attendance requirements, and fulfill the expectations outlined in the audition forms. The student s continued participation in the production(s) is dependent upon these conditions. Auditions are open to any student who meets the audition requirements. I have read and understand all the information included in the Parent Form. Student name Parent s Digital Signature 2

Release Form Publicity Release By signing below, you give Xavier Theatre permission to include your child s name, likeness or photograph in publicity releases, promotional photographs and website/social media postings. I have read and understand the publicity release. Licensing Rights Acknowledgement By digitally signing below, you acknowledge that any online dissemination of images, sound and/or video taken during an Academy dress rehearsal or performance by audience members or performers is prohibited by our licensing contract, unless express consent by the Academy staff has been given. Parents and/or students found to be in violation of Academy's show license face penalties up to, and including, dismissal from a production with no refund. Academy show licensing grants us the right to record a performance that can be purchased by parents on DVD or Blu-ray through the vendor. These videos also cannot be disseminated in any form online. I have read and understand the licensing rights terms and conditions. Student Availability Form It is very important that we are aware of the student s availability during scheduled rehearsals and performances. Roles will be offered with student availability in mind. All students and parents are expected to honor the time commitment indicated on this form. Our expectation is that Academy attendance should take precedence over conflicts (such as sports tournaments) that arise after a student has committed to this program. Changes in availability for any reason after this form is submitted (other than unanticipated emergencies) negates a performer s agreement with the academy and may result in reduction of the student s participation in the show(s) or dismissal from the cast/crew with no refund. Unanticipated emergencies that significantly impact a student s availability and the integrity of the production(s) may also result in reduction or reassignment of the student s participation in the show(s). I assert that my student is available for rehearsals/performances of the production(s) in which he/she is interested at any time scheduled on the calendar that has not been indicated by me as unavailable. I understand that rehearsals/performances for which my student is available will be the priority should a conflict with another activity arise. I am aware that absences from rehearsals/performances that have not been indicated below must be of an emergency nature (illness, death in the family, or other extenuating circumstance) and that failure to honor the availability provided at auditions may result in a change to my child s participation in the production(s) or dismissal with no refund. Full Availability For the show(s) in which your child is interested, is your child available for ALL rehearsal and performance dates and for the full duration of the times listed in the calendar? If you indicated Yes, the calendar on the next page should have no conflicts listed for the show(s) in which your child is interested. Student name Parent s Digital Signature 5
