2 Observed raw fish stored over ready-to-eat vegetables in main kitchen reach-in cooler (improper vertical storage). CA: Food items must be stored in the correct descending order based on each items minimum final cook temperature. Observed failure to have a food service permit posted that is current and not past it's annual renewal date. CA: Must obtain. Advised that these items be removed for cleaning. Observed gallon of skim milk stored in reach in cooler in the front food service area marked with an expiration date of 7/10/2018. Advised that milk must be discarded and that expiration dates must be checked daily. Observed no thermometers in most of the reach in coolers and walk in cooler/freezer in the main kitchen area. Thermometers must be located in each of the units and be easily visible at all times. Observed several wiping cloths stored on prep area counters and stored directly on shelves. Advised that all wiping cloths not in use must be stored in an active sanitizing solution.

CA: All hand sinks must be stocked with soap and paper towels. All areas where violation was observed were corrected onsite. Observed equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Items were just washed and rinsed. CA: Equipment used for food service must be washed, rinsed, and sanitized and then allowed to air dry. Observed items stored in self service prep top coolers failing to maintain temperatures of 41 degrees F or lower (see temp. observations). CA: Items were discarded onsite. All cold held items must maintain temperatures of 41 degrees or lower or facility must begin maintaining time as a public health control. Observed potentially hazardous chicken not held at 135 degrees Fahrenheit or above at three different hot holding units. CA: After items have been cooked to their required final cooked temperature, food intended for hot holding must maintain temperatures of 135 degrees F or higher. All items in violation were reheated to 165 degrees F or higher. Observed ready-to-eat potentially hazardous food held more than 24 hours not date marked. CA: Items meeting the parameters previously stated must be date marked. Observed chlorine sanitizing solution exceeding the maximum concentration allowed. CA: Chlorine sanitizer must have a concentration between 50 and 100 parts per million. No date was provided for a follow-up inspections. CA All cold held items must maintain temperatures of 41 degrees F or lower. Inspector was informed that the unit is unable to maintain required food cold holding temperatures. Legal notice issued for preptop cooler to be repaired. Items with time to cool were underwent rapid cooling procedures. Items without time to cool were discarded onsite. Observed boxes of food stored on the floor of walk-in freezer. CA: All items must be stored at least 6 inches off the ground. Observed door to food preparation area left open with no screening or air curtain. CA: Doors exiting main kitchen and leading to outdoors must remain closed unless a fine mesh enclosure or an air curtain is present so as to prevent the entry of pests. Door was closed onsite. Observed build up of debris in the ice machine in the main kitchen area. Advised that it must be cleaned frequently enough to prevent the buildup of debris within the machine. Observed wiping cloths stored between equipment in the main kitchen area. Wiping cloths must be stored in sanitizer buckets with sanitizing solution at all times. Observed tiles missing in front of the grill area in the main kitchen making the floor no longer easily cleanable. Advised that floor needs to be repaired. Observed multiple flies in the main kitchen and front food service area around the bar. Advised that more pest control devices may need to be installed to control flies.
