Volusia County Community Assistance Summer Camp Scholarship Application Information 2015 Summer Camp scholarship applications will be released to the public on April 6, 2015. Approved summer camp sites will be available to the public after March 19, 2015. Required Documents: Parent s photo ID Social Security cards for the child or children applying Proof of household size. This may include one of the following: o SNAP award letter (dated within past 30 days) o 2014 tax return o Current lease or rental agreement o Public housing verification letter which indicates household size Proof of all household income for the past 30 days. This may include the following: o Paystubs o TANF o Child Support o Current Social Security award letter o Unemployment benefits o Worker s compensation o Pension o Household assistance o Student Financial Aid Supplemental documentation needed may include: o Self-employed 2014 tax return Work calendar o Public housing Income worksheet from the appropriate housing authority (dated within the past 12 months) o Homeless shelter 1
Special Situations: Verification of housing status (dated within the past 12 months) Client income o No income and living with someone Foster Care Child Eligibility: Age Household income for the family member Residency verification o Guardian must provide documentation from the state or agency verifying they are the foster care parent of the child. Residency o Children ages 5, who have completed Kindergarten, and children age 6-12 (must be entering 1 st through 6 th grade) o Children up to age 17 for City of Ormond Beach Camp T-Rec (Special Needs Only) o Children must live and attend school in Volusia County Income Eligibility o Clients must be between 0-200% of the federal poverty guideline.
