The below questions, responses, additions, clarifications, changes, and/or attachment(s) shall amend FDVA-ITB-16-008B. Addition: Specifications for SNO 6084R 2MEGAPIXEL Full HD Network IR Bullet Camera or approved equal. (Question #15 and FDVA Response) 1. Vendor Question: Based on the drawings/specifications we noticed that using the emergency circuit at 480 VAC has been specified on the specifications page 97, but the power panels that were shown to us were a 120/208VAC panel. Can you confirm which panels we are to use and that the proper voltage for the emergency circuit is on those panels? The facility has both 277/480V and 120/208V load centers that are connected to the emergency generator. There is available space for additional circuits in both Room 034 (Lobby Electrical Room) and C193 (Loading Dock Electrical Room) on either the 277/480V or 120/208V panel. The panels are within 5 feet of each other. 2. Vendor Question: On Page 108 for the Parking Lot Barrier Schedule it lists 460VAC for the primary input voltage, but in specifications it lists 480VAC in the specifications on page 68. Which input voltage will be required? The voltages are essentially the same; 480V is considered the distribution voltage and 460V is the utilization voltage. 3. Vendor Question: Also, for the flood lights at the automatic swing gate, and the two front entrance/exit gates referenced on pages 106, 109, 110 and 111; 277 VAC has been specified for the light on the table on page 109 for the Lithonia lights, and we would like to know if we are permitted to use the multi-tap in the light to run 120V to the gates and other components that require power. The vendor may use multi-tap lighting connections to provide compatibility with the supply voltage.

4. Vendor Question: As a VE proposition we think that using common light pole instead of two separate light poles for both cameras in the middle of entry/exit gates would be more than sufficient for both lighting and camera view, and reduce costs for the FDVA. Utilizing a common pole will also give you a view of both lanes driver side angles. Will that be allowable? The awarded contractor shall install two (2) separate light poles. 5. Vendor Question: We will need to mount a NEMA box enclosure out at the gate locations on referenced on pages 106, 109, 110, and 111 in order to protect the electronic components for the cameras, and power runs from the elements. Will it be required to provide a Climate Controlled NEMA Enclosure to ensure that equipment will not be overheated? A Climate Controlled NEMA Enclosure is not a requirement. 6. Vendor Question: We also took measurements of the areas you would like to mount cameras out at the gates as specified on pages 106, 107, 109, 110, and 111 and the distance exceeds the allowable 300 feet for a Power over Ethernet connection. The front entrance/exit gates are ~450 feet from the building and the back swing gate would be ~850 feet from the building. In our expert opinion, you need to run fiber cable to these two locations, or Recording Device will not receive communication from the cameras at these locations. What type of media converters would the FDVA like specified for this? What type of fiber cable is going to be specified by the FDVA? Would FDVA like to have spare pairs of fiber out at these locations in the event you would like to add additional communication equipment in the future? If so, how many pairs. ITB - Section V Statement of Work 17. h) Cable/Wiring. The Contractor shall provide new wiring, cable, fiber optic, transceivers, and other ancillary items required for the complete installation of the CCTV, remote gate operators, lighting and voice communication as required. Existing communication wiring shall not be used to supplement the LAN or communications systems. The awarded contractor shall provide FDVA two (2) spare pairs of fiber at the locations identified above. 7. Vendor Question: How much storage space is going to be required for the Recording Device? We noticed on the Specifications pages 86 and 87 that the recorder has 2 Terabytes built in, but nothing is noted for how many frames per second you want the cameras recorded at, and how many days you want the video stored for. ITB - Section V Statement of Work 17. e) Closed Circuit Television System 5) - Two, Sixty-Four (64) channel Network Video Recorders (NVR) capable of recording a minimum of 30 consecutive days of video at 15 frames per second from all cameras on the system. NVR shall support a viewing monitor(s) or Contractor shall provide the necessary equipment to allow a viewing monitor(s) to be used.

8. Vendor Question: We would also need to know how high the traffic is in certain areas throughout the day in order to understand how often the cameras would be recording. We are unable to provide any information regarding traffic volume. 9. Vendor Question: In addition to this we have concerns that no Power over Ethernet switches have been specified in order to accommodate the bandwidth required for the data throughput from the cameras to the Recording Devices. Fiber cable will also need to be run between the internal closets to accommodate the bandwidth needed to the NVR. ITB - Section V - 17. Site Work. Contractor shall comply with all Specifications and Drawings (Attachments C and D ). The Contractor shall furnish and install a Vehicle Boom Barrier System with access control for the main entrance and exit at the front parking area; gate and fencing, gate operator and access control at the rear delivery/employee parking area; and a CCTV system consisting of 2.1 Megapixel (nominal) fixed/variable focus IP Cameras, Pan, Tilt, Zoom (PTZ) Camera(s), network cabling, encoders, network video recorder(s) (NVR), Ethernet switches and/or routers and servers. Also see FDVA response to Question #6. 10. Vendor Question: What Power over Ethernet Switches with fiber capability are going to be specified for the exterior camera locations, and interior Communications closets? The awarded contractor shall provide compatible equipment of their choice for items not specified. 11. Vendor Question: On Converting the existing analog system cameras over to IP, what analog to IP converters are being specified for this project? The awarded contractor shall provide compatible equipment of their choice for items not specified. 12. Vendor Question: What type of Computer is being specified for the workstation referenced on page 107 with two 40 monitors? See ITB Section V 17. e) 7). Personal Computer (PC) specifications. 13. Vendor Question: Will the 19 inch monitors on page 107 need full functionality if an event happens and needs to be addressed right away?

The 19 monitors shall be view only. 14. Vendor Question: Reference Page 36 e) Closed Circuit Television System: A closed circuit television (CCTV) system with audio shall be installed in the locations designated to monitor the interior and exterior of the facility. PTZ and box cameras located on the exterior shall be video only. The system requirements are as follows: 1) Sixty-Two (62) high resolution, 2.1 Megapixel minimum, color with infrared night vision, vandal resistant, mini-dome cameras at the locations specified by the Contract Manager. The cameras shall be furnished complete with powered, varifocal lenses and 64GB Micro SDXC Class 10 memory card. During the pre-bid walk-through, the interior camera locations were identified as listed above, but was not identify whether to install audio as identified above. Interior cameras shall be audio/video (record/view). Exterior cameras shall be video only. 15. Vendor Question: Point of clarification. During the pre-bid walk-through, the exterior box cameras were allowed to be substituted with either dome and or bullet style 2.1 Megapixel minimum, color with infrared night vision, vandal resistant. The awarded contractor shall install SNO 6084R 2MEGAPIXEL Full HD Network IR Bullet Camera or approved equal. Specifications for camera attached separately to Addendum No. 1. 16. Vendor Question: 5) Two, Sixty-Four (64) channel Network Video Recorders (NVR) capable of recording a minimum of 30 consecutive days of video at 15 frames per second from all cameras on the system. NVR shall support a viewing monitor(s) or Contractor shall provide the necessary equipment to allow a viewing monitor(s) to be used. Can contractor install (4) NVR with the capacity of 32 channels, each which will meet or exceed the existing specification of Two (2) 64 channel? The NVR s shall be as specified in the ITB. 17. Vendor Question: Reference page 39. 26. Invoicing and Payment. Invoicing: Contractor shall submit invoices. Can contractor submit a drawl request as follows: First request 50% of total once all interior security cameras devices have been installed terminated and tested to include master control computer, and monitors. With balance. Upon completion. The awarded contractor shall be paid in full at FDVA Contract Manager s full acceptance and approval of the completed project. 18. Vendor Question: Is the MFMP transaction fee 1.0% or.7%? a. Section II, #14. Transaction Fee (page 9).

ITB - Section II, General Contract Conditions, Page 9, 14. Transaction Fee is hereby amended to all payments shall be assessed a Transaction Fee of seven tenths of one percent (0.7%), which the Contractor shall pay to the State, unless exempt pursuant to 60A-1.032, F.A.C. 19. Vendor Question: Can the project Calendar Days of 180 days start from a date given by a Notice to Proceed after all bonds, permits and insurance requirements are submitted to FDVA and not from when the contract is signed? FDVA requires specified services to be completed to the full satisfaction and acceptance of FDVA and any applicable authorities having jurisdiction, within one-hundred-eighty (180) calendar days from the date of Agreement s full execution. 20. Vendor Question: Can the FDVA verify with the city of Lake City on WHAT permits will be required and what documentation will be required to pull the permits? 21. Vendor Question: Will the contractor be responsible for the cost of creating and submitting signed and sealed drawings to get the permit for the project? 22. Vendor Question: When I spoke with Bob Hathcox (Growth Management Director) he indicated that a GC or CBC license may be required for the gates and vehicle barriers that are a part of the project. ITB - Section IV Special Instructions to Respondents - 10. Qualifications. Award of the Agreement, in all respects of this solicitation and any issued addendum, shall be made to the Respondent whose bid is determined to be the lowest responsive, responsible bid, a determination that shall be made solely at the discretion of FDVA. The Respondent affirms and declares that Respondent has: c) All federal, state and local registrations, licenses, certifications, and permits legally required to perform and complete the services as called for herein; including but not limited to any other related agreements.

23. Vendor Question: Also, we will need to submit 2 sets of signed and sealed (by an architect or engineer) drawings to obtain the permit from the City. 24. Vendor Question: Can an amount be given by the FDVA for an allowance to include the cost of the permit, signed drawings and other requirements by the AHJ? 25. Vendor Question: Are CAD drawings and the cost to create them required by the contractor for the as-built drawings? Section V, #22. Drawings (page 38). Yes. 26. Vendor Question: Can the payment schedule be changed from a one-time payment to a progress billing or a multiple payment project? Payment schedule suggestion (using the work descriptions on pages 35, 36 and 37); Payment 1 Bond, Permits, General Conditions. 27. Equivalent Submittals: Equivalent Submittal Requests received from five (5) Contractors by the date and time as specified in the Invitation to Bid (ITB) were reviewed by FDVA s Contract Manager and Alternate Contract Manager. Sign and return the Addendum Acknowledgment Form with complete bid package. Failure to do so may result in the disqualification of your bid.
