25 6 th Grade Music Band, Chorus and Orchestra are daily classes scheduled during the Middle School day. A letter grade is assigned each quarter as instrumental and vocal music are part of the curriculum. Class Options: Band Chorus Orchestra Combo (Band& Chorus or Orchestra & Chorus)

Fifth grade students are being given the opportunity to select band, chorus, or orchestra as an elective course for their 6 th grade year. Physical Education and Health combine to make a required, full-year course for 6 th grade students. Computers I is a semester (1/2 year) required course. Students selecting Option 3 will need to take the required Computers II as well during their 6 th grade year. INDIAN PRAIRIE SCHOOL DISTRICT TH GRADE ELECTIVE REGISTRATION FORM Return to 5 th grade teacher by January 31, 2014 NAME Option 1 Band, Chorus, or Orchestra Students If Band, Chorus, or Orchestra is one of your choices, indicate that choice by placing an X on the respective line. Combination courses (ie Band/Chorus or Orchestra/Chorus combo) may be selected and will be scheduled on an alternate day basis. If you are interested in a combination course, place an X on the lines of your two choices. If you do not wish to be in Band, Orchestra, or Chorus, do not mark this Option. Band Orchestra Chorus Instrument Signature of Instrumental Music Teacher: Option 1: Band/Orchestra/Chorus ONLY Parent Signature: Date: (Signature indicates approval of child s selection) Option 2 Foreign Language Students While the comprehensive twoyear Foreign Language courses do not begin until 7 th grade, students need to indicate that they wish to participate in the middle school foreign language program. The following will be the language offerings listed on their 7 th grade registration form. French I Spanish I Option 2: Foreign Language ONLY Option 3 Band, Chorus, or Orchestra Students + Foreign Language Students While the comprehensive, two year foreign language courses do not begin until 7 th grade, any student who wishes to take Foreign Language plus Band, Chorus, or Orchestra must follow a prescribed path in order to meet the computer requirements for District 204 middle school students. These 6 th grade students must take two semesters of computers, and consequently, their schedules do not permit them to take any of our other exploratory or elective offerings. Band Orchestra Chorus Instrument Signature of Instrumental Music Teacher: Option 3: Band, Chorus, Orchestra + Foreign Language Option 4 Students Not Wishing to Take Foreign Language or BCO Sixth grade students who do not take Band, Chorus, or Orchestra have four, nineweek rotations of Music, Art, Design & Modeling, and Family & Consumer Science. In addition, these students will also have nine weeks of Foreign Language Experience (FLEX), and nine weeks of Study Skills. Option 4: Students Not Wishing to Take Foreign Language or BCO
