5 49. All information in the tender document must be supported with product data sheet. All information asked for must be provided in the same order as in the specification. Incomplete and ambiguous information will not be accepted. 50. The machine should be provided with 5 years guarantee including spares & service. 51. A certificate stating that spare parts will be available for 10 years from the date of supply to ensure proper functioning of machine (Rate list of the spares valid for 10 years should also be submitted). 52. Annual comprehensive maintenance contract rates for 10 years after the expiry of guarantee period should also be quoted. 53. The rates should be quoted FOR up to Department of Cardiovascular & Thoracic Surgery, Institute of Medical Sciences, Banaras Hindu University, Varanasi , (U.P.). 54.
