Please abide by accident prevention laws where applicable and pay special heed to the printed instructions and information on the unit itself. The unit may be used in medical areas only if these rooms are equipped according to VDE 0107 norms or the equivalent (Association of German Electrotechnical Engineers). Always disconnect all main plugs from their power outlets before carrying out maintenance or cleaning work. Disconnect the main plug at once if you notice smoke, sparks, or unusual sounds coming from the unit. Do not use the unit again until the problem has been corrected by our service personnel. Do not connect electrical plugs and sockets by main force. If it is not possible to connect them, verify whether the plug is correct for the socket. If you find damage in either the plug or the socket, have them repaired by our service personnel. Do not disconnect electric plugs from their sockets by pulling on the cable, but rather on the plug. Additional equipment which is connected to the analog or digital interfaces of the unit must have been shown to meet European Union norms or IEC specifications. Moreover, all configurations must meet the requirements of IEC Systems Norm No Under no circumstances may integrated electric medical systems be created by coupling the Binoptometer 3 to non-medical electric devices (e.g. data processing equipment) in such a way that patient safety is below that required by IEC Norm No A safety disconnection device must be provided if it is possible that such a coupling might lead to higher leakage current values than those which are permissible. Do not use the equipment named in the Standard Equipment List in situations: -Where there is danger of explosion. -Where flammable anesthetics or volatile solvents such as alcohol, benzene or the like are present. Do not store or use the unit in damp rooms. Avoid placing the unit near dripping, gushing, or splashing water, and make certain that no fluid can enter the unit. 7 Instruction Manual Binoptometer 3 Page 7 5 Appropriate Use of the Unit This Binoptometer 3 is designed for the purposes described in this Instruction Manual only. It is designed for: Measuring visual acuity Testing binocular vision Testing color perception they will work with the unit in an appropriate manner. Operate the unit only with original parts and accessories delivered by us and in a technically flawless condition. Please abide by the safety precautions given above. This unit may be used only by persons whose training and practical experience ensure that 5.1 Warning Signs and Instructions on the Unit Caution! Disconnect the main plug before opening the unit! Please read the accompanying instructions! Type B applied part 6 Putting into Operation 6.1 First Use of the Unit 1. Pull out the protective foam shipping pad from under the viewing mirror control knob. We recommend a table whose height can be adjusted to the patient's physical size. 2. Select a place of installation. The Binoptometer 3 must rest on a level surface to ensure that it is adequately ventilated through the openings in its base. Select a place of installation where the patient is not exposed to strong light sources, since this could lead to distracting reflections. The unit can be installed approx. 1-2 m in front of a wall. It can also be installed in front of a window, though not with exposure to direct sunlight or excessive backlighting. If necessary, a dark backdrop should be hung in front of the window for the patient to view the test fields against. 3. Connect the Control panel, the Comfort Control Unit or the serial cable of the PC to socket (No.12, see page 8). The plug of the Control panel locks automatically when inserted. 4. Connect the mains cable (socket No.13). 5. Turn on the mains switch (No.11); the pilot lamp now lights up. 6. The unit is ready for use after a brief, automatic self-check.

Page 8 Instruction Manual Binoptometer 3 A proper container is required to protect the unit against damage during transport. We recommend the carrying bag which is available as an accessory (See page 50 for the order No.) The Binoptometer 3 contains optic components which may become clouded by condensation after abrupt temperature changes from cold to warm. For this reason the unit should not be kept for longer periods of time in a vehicle during the cold season of the year. If this is unavoidable, however, the unit should be warmed for 1-2 hours in a normally heated room before being used. 6.2 Instructions for Transportation Do not carry the Comfort Control Unit by holding it at the grey cover. The cover is not permanently fixed as it must be removed to add new printer paper. If the Comfort Control Unit is held by the grey cover, the unit may accidentally detach from the grey cover and fall off. If you use the carrying bag, you have to lay in the Comfort Control Unit with the upside down. The Binoptometer 3 is a PD-independent, freeview unit for testing visual acuity. "Free-view" means that the tests appear under natural visual conditions in free space. Mechanical accommodation is eliminated for the most part, since the eyes accommodate and converge as in normal vision. An integrated neutral density filter ensures that both the test panel and the visual field adhere to DIN test requirements. Wearers of spectacles with multifocal and progressive lenses are easily examined thanks to a swivelling mirror in the viewing aperture. The newly designed viewing section permits the test person to keep his head in a Components of the Unit: 7 Description of Unit comfortable position regardless of the angle of the viewing mirror. The glare shields provide protection from irritating light reflections. The unit automatically carries out the proper settings for each test, such as occlusion of the right or left eye and test distance. The test of visual acuity is conducted among other things with the Landolt ring acc. to EN ISO Figures are also included in the test program (in conformity with EN ISO 8597). Moreover, binocular functions and color perception can also be tested. 1 Forehead rest 2 Markings for height of patient eyes 3 Viewing aperture 4 Front of the housing 5 Control knob for swivelling the viewing aperture mirror 6 Ventilation openings 7 Light admission window 8 Back of the housing 9 Rating plate 10 Recessed grips for carrying the unit 11 Mains switch 12 Interface for Control panel, Comfort Control Unit or PC 13 Coupling socket and fuse assembly 14 Locking screws with caps for back of housing

Page 10 Instruction Manual Binoptometer 3 The Landolt ring, a standardized optotype acc. to EN ISO 8596, is presented in 8 positions, that is: with the gap directly at the top, bottom, right or left, or with the gap at 45 from the direct positions, i.e. top right, top left, bottom right, or bottom left. 8 Description of Optotypes and Tests 8.1 Large Landolt Rings (Tests 1-3) The optotypes are present in different levels of visual acuity. Visual acuity is expressed as the reciprocal of the angle of the gap in the Landolt ring measured in angular minutes. These tests use the larger optotypes. The following visual acuity levels are present in each test field: V 0,2-0,4-0,5 For general testing of vision, 5 optotypes acc. to DIN No T5 are provided for each level of visual acuity. The visual requirements are fulfilled when each eye correctly recognizes at least 3 optotypes in the set of 5 optotypes. These tests use the small Landolt rings. Five optotypes are provided for each level of visual acuity. 8.2 Small Landolt Rings (Tests 4-8) The following visual acuity levels are present in each test field: V 0,5-0,6-0,7-0,8-1,0 1,25

Instruction Manual Binoptometer 3 Page 11 The Stereo test consists of a non-polarized dot in the middle, with double lines above it, to the right and left of it, and below it. Of these double lines, the right line is visible to the left eye and the left line to the right eye. The resulting transverse disparity has the effect that the lines appear to lie spatially in front of the dot. For purposes of recognition, the lines are provided with non-polarized numbers. To the test person the lines appear to lie at different depths. The dot is furthest away, and the 9 is closest. 8.3 Stereo Test (Test 9) No. "3" lies ca. 80 angular seconds in front of "12". No. "9" lies ca. 160 angular seconds in front of "6". The polarized lines are compared by the examinee with the level of the dot: No. "12" lies ca. 600 angular seconds in front of the dot. This illustration shows the test in its non-polarized state (Aperture: BIN). Stereo test in the polarized state as seen by the test person (differences not true to scale).

Page 12 Instruction Manual Binoptometer 3 The phoria test is used to examine possible disparities in position between the test person s eyes. It permits identification of horizontal as well as vertical heterophoria. The large rectangle is visible to the left eye, while the small square is visible to the right eye. The square is positioned asymmetrically in the rectangle. The distances are 2.25 prism diopters towards esophoria and 3.75 prism diopters towards exophoria, i.e. the overall width of the rectangle is 6 prism diopters (1 prism diopter = 1 cm/m). With respect to height, the square is positioned symmetrically in the rectangle. The overall height of the rectangle is 3 prism diopters. The four dots making up a trapezoid stimulate the test person s binocular focus. 8.4 Phoria Test (Test 10) Test persons with non-pathological heterophoria see the square inside the rectangle. Test persons with pathological heterophoria see the square outside the rectangle or they may see only the rectangle and the dots or only the square and the dots. In the latter case it is advisable to have an ophthalmologist examine the test person for latent or manifest strabismus. Patients with monocular sight see either only the rectangle and the 4 small dots or only the square and the 4 dots. In more severe cases of strabismus the test person sees the 4 outer dots doubled, i.e. he sees more than 4, usually 8 dots. Visual impression in non-pathological heterophoria Left Eye Right Eye binocular binocular Binocular impression in pathological heterophoria Esophoria (more than 3 cm/m) Exophoria (more than 4 cm/m) Hypophoria RE or Hyperphoria LE (more than 2 cm/m) Hyperphoria RE or Hypophoria LE (more than 2 cm/m)

Instruction Manual Binoptometer 3 Page Large Landolt Rings acc.to the German Driving License Ordinance (FeV) (Test 11) This test field contains 2 Landolt rings corresponding to a visual acuity level of 0,05. The German Driving License Ordinance FeV, which took effect on 1/1/1999, requires that sight tests on candidates for any of the following driving license categories should demonstrate a minimum uncorrected central photopic visual acuity of 0,05 for either eye : C, C1, CE, C1E, D, D1, DE, D1E and passenger transport license. A distinction is made between congenital and acquired impairments in color perception. Congenital impairments in color perception are relatively common among men (8%), less so among women (only 0.4%). They usually involve so-called "red-green defects", whereas congenital blue-yellow defects are very rare. The Ishihara plates are the best known of the many color perception tests available today. 8.6 Color Vision Test (Tests 12 15) Clinical studies have shown that they screen congenital red-green-color impairments very accurately. The Binoptometer 3 offers the four most effective Ishihara plates; their numbers can be correctly identified only by patients with normal color vision or at the most very mild color vision defects. Noticeable hesitation on the part of the patient is often a sign of such mild defects. Subjective Visual Perception Test No. Trichromat Red-Green-Impairment 12 74* 71, 21 or none Normally none 14 2 Normally none or none * The number 4 is often seen at first as a 1, even by trichromats. On closer view, however, they recognize the 4. A test person who does not recognize all four tests with certainty is considered as being outside the visual norm. Detailed examination is called for in this case. The Binoptometer color test serves only to detect congenital red-green color vision impairment; it does not permit classification or quantification. An anomaloscope is recommended for this purpose. The used colour slides have been reproduced from Ishihara`s Tests for Colour Deficiency, published by KANEHARA & CO., LTD, located at Tokyo in Japan. For more extensive examinations you can get a complete set of the original plates.

Page 14 Instruction Manual Binoptometer 3 These optotypes are used for the sight test stipulated by the German Driving License Ordinance (FeV) for applicants for road vehicle driving licenses. Each test field tests the following visual acuity levels: 0,32 0,7 0,8 1,0 The optotypes in line 1 (visual acuity level 0,32) are for practising. They are arranged similarly in both test fields, 2 straight and 2 slanted. 8.7 Landolt Rings FeV (Tests 16, 17) These practice optotypes are for familiarizing the test person with the test procedure. In the sight test for applicants for road vehicle driving licenses DIN requires that 10 optotypes are used for each visual acuity level. The test person s vision is considered sufficient if he or she is able to recognize 6 out of 10 optotypes. FeV specifies the visual acuity level required for each driving license category. These tests employ figures adapted to the Landolt Ring in accordance with EN ISO Five figures are used: 3,5,6,8 and 9. Each test field tests the following visual acuity levels: 0,5 0,6 0,7 0,8 1,0 1, Figures (Tests 18-20) There are 5 optotypes for each visual acuity level. The figures tests are not used by any of the standard programs. They can either be selected via the program ALL or defined in the USER program.

Instruction Manual Binoptometer 3 Page 15 Any preceding adaptive disturbance must have subsided when the sight test starts. If the test person wears spectacles, these must be examined for damage or soiling. The test person should be instructed about the type and arrangement of optotypes being used. Tell the test person that he or she is required to read the optotypes line by line (in the visual acuity test). 9 Test Preparations 9.1 Preparing the Test Person 9.2 Instructing the Test Person The height of the device should be so adjusted that the test person can sit comfortably. This can be achieved by means of a vertically adjustable table, for example. There are various ways of describing the orientation of the Landolt rings. You can show the test person the following table: Top Left Half Past Ten Top Twelve O Clock Top Right Half Past One Left Nine O Clock Right Three O Clock Bottom Left Half Past Eight Bottom Six O Clock Bottom Right Half Past Four

Page 16 Instruction Manual Binoptometer 3 The turning knob on the side permits continuous adjustment of the viewing mirror. Normally the mirror will be in position F (far). However, it can be adjusted continuously all the way to position N (near) to permit test persons wearing reading, multifocal or progressive spectacles to achieve perfect focus and keep their head in a comfortable position when viewing optotypes as at an intermediate or short distance. The intermediate values on the scale give the angular degrees from position F. They can be helpful in repeat examinations. 10 Operation 10.1 Controls on the Binoptometer Adjusting the Viewing Mirror The values are not to be taken as absolute, since the true angle will differ according the shape of the test person s head. We recommend allowing the test person to adjust the viewing mirror himself, since he will be able to judge best when his view through the reading segment of his spectacles is sharpest. Make certain that the test person s eyes are always approximately on a level with the markings on the device. Otherwise the test field may be cut off and the test person may not be able to see all the optotypes. Normal position of the viewing mirror Example of device setting for multifocal spectacles and examination of near vision

Instruction Manual Binoptometer 3 Page Operating with the Control panel Description of the Function After the Binoptometer is turned on it automatically selects the G37 test. If you wish to select another test program, press the PROGRAM key until the desired program is displayed. The following programs can be selected: G37, G25, FeV, USER, and ALL. After selection of a program, the first test in the displayed program is activated, and the examination can begin. Pressing TEST> and TEST< In the ALL program causes the test characters of the test disk to advance or go back character by character from Test 1 to Test 20. Distance and the eye remain as before at the settings of the USER program. However, they can be changed at any time (cf. the following passages). The changes remain unchanged in the next test. In all other programs, pressing TEST> or TEST< causes the following or the preceding test to be presented in specific increments. The smaller optotypes are presented first, in order to reduce the time of examination. If the patient is unable to read the optotypes, the test with larger optotypes appears after the key is pressed. Pressing this key again causes the previous test with smaller optotypes to reappear. As an exception to the default settings of the program, distance and the eye occluder are adjustable. In the next test, however, the standard value is set again. Pressing the DIST.< or DIST.> keys reduces or increases the test distance. Possible distances are: meters and Infinity. Four settings are available for the eye occluder: R - Examination of the right eye. L - Examination of the left eye. BIN - Examination of both eyes. ST - Examination of both eyes through polarizing filters. Pressing the R/L/BIN key advances to the next position in this series, so that R begins again after ST. When the program reaches the final test, it can be started again with the START/STOP key. The START/STOP key can also be used to start over again during the program. Helpful hint: After a specific period the bulb switches automatically to standby mode in order to prolong its service life. Press any key of the control unit (e.g. F1 or TEST >) to reactivate it.

Instruction Manual Binoptometer 3 Page 19 A user-defined test sequence can be created from each program. This is best done in the "ALL" program, because only there the test numbers will be displayed. At all other programs only the program step number will be displayed User-Defined Test Sequences As long as the unit has not been turned off, saving new tests causes them to be appended to the already existing test sequence (provided that no more than 15 tests are saved). When the unit was turned off, the old test sequence will be overwritten. 1. Press the MEMORY key. "MEMORY RELEASE" appears briefly in the screen display. 2. Now select the desired test, modify it correspondingly (distance, eye). 3. Incorporate the test into the test sequence with TEST SELECT. "MEMORY X" is briefly displayed, where "X" is the memory space occupied in each case. After being saved, the test sequence can now be carried out in the program "USER" at any time. Only the program step numbers are shown in each case. For drawing up an USER test form sheet (see next chapter), you should make a note of the programmed test numbers. 4. Proceed in the same way with other tests. A maximum of 15 tests can be selected. 5. After all desired tests have been selected, save the test sequence by pressing the MEMORY key. "PROGRAM MEMORY" is displayed briefly Using Test Sheets Test sheets for Binoptometer 3 (order numbers see page 50) are available for the examination procedures s as well as for user programs. Each test field description is preceded by the number of the program step shown in the display. In test sheet 59669, which is for examinations acc. to G37/G25, the tests for G37 are given on the front and those for G25 on the rear side. Test sheet is for the sight test for road vehicle drivers in accordance with FeV (German Driving License Ordinance). On the next page you see a reduced image of the test sheets. The test sheets are only available in German language. USER test sheets are for compiling user-defined (USER-) programs. For this purpose a test sheet has been included at the end of this Instruction Manual for you to copy (see Appendix). You can copy this test sheet and cut out the tests you want to program. Stick the tests onto a second sheet in a column in the desired order and enter the necessary settings (examined eye, distance) and the running program step numbers. In test fields with optotypes you can decide whether you want recognized or not recognized Landolt rings or figures to be marked. In the color vision, phoria, and stereo tests just cross the desired option.

Restoring from a backup data set can be carried out according to different criteria: "Delete existing data first" Caution! This function deletes all presently stored examination data of the patient before restoring data from the designated data storage medium. After the restoration, therefore, only those examinations are present which are already found on the backup data storage medium. "Add examination data" This function adds the examination data of the backup storage medium to the already existing examination data of the patient. Use the [Restore] button to start the restoration of data.

Instruction Manual Binoptometer 3 Page How to Change Settings Activating the [Settings] button causes the "Change settings" menu to appear. This can be used to adapt the patient data management system to your own preferences: Language: Use this function to set the language used by the program for output (German, English, French, Italian, Spanish or Japanese). Date format: Used to change the date display. The following sequences (with punctuation) can be selected: Day/Month/Year (DMY), Month/Day/Year (MDY), or Year/Month/Day (YMD) Sort The patient list can be sorted either by patient names or ID numbers. Sort the list by name if you customarily use patient names in searching for patients. On the other hand, use ID numbers if you customarily search in this way. Date Use this box to change the system time and date if necessary. Automatic Backup This function has no significance for the Binoptometer 3. Import / Export The backup directory in which data backup sets will be located must be selected here. You also can set up the Serial Port where the Comfort Control Unit is connected to the PC. The [Use] button implements your choices for the continuing course of the program. However, the previously stored settings are again loaded when the program is started again. The [Save] button can be used to make your choices permanent. They will then be implemented every time the program is started until they are changed and again stored with [Save]. The [Cancel] button rejects the changes without implementing them and closes this window.

Page 42 Instruction Manual Binoptometer The Examination Program Start the examination program by selecting a patient from the patient list and then clicking the [Binoptometer 3] button. This examination program is the PC version of the Comfort Control Unit and is nearly identical to it in function (see 10.3, page 21) Differences from the Comfort Control Unit In order to mark the elements of the individual tests on any line, simply click the visual acuity value of that line with the mouse or press the corresponding line number on the keyboard. The Hotkeys Plus and Minus correspond to the + and - keys of the keyboard. The function (larger/smaller Landolt rings) is allocated to the [Enter], g key and x key of the numerical keyboard.

Instruction Manual Binoptometer 3 Page 43 Instead of "Display" (as in the Comfort Control Unit), the "Settings" menu contains the Settings "System" dialog box. Here you make the following selections: Select the date format in the "Format of date" area as desired (see , page 41). Use the "Serial Interface" list to select the interface through which the Binoptometer will be connected to your PC. Use the "Language" field to select the language in which you wish to operate the program Sample Printouts Standard printout (reduced in size) Detail printout (reduced in size)
