3 1. Preface The year 2002 is a historical year for both Korea and Japan because the World Cup Soccer event was successfully co-hosted by the two countries for the first time in history. This was when our medical team joined the Hyeonhea/Genkai project, which was established by cooperation between governments, academic institutions, and companies, in order to promote mutual relationship in such fields as culture, education, business and sightseeing using broadband Internet via the Korea-Japan submarine optical cable. It was both fortuitous and a kind of coincidence that the medical team started to work on this project. Hanyang University in Seoul was a common friend for both engineering and medical teams. In addition, three surgeons (HS Han, BI Moon, and YW Kim) had been invited to a local surgical meeting in Fukuoka in the same year. All these doctors are now key counterparts leading the project. This joint project seems to be best suited to medical use. In contrast to the conventional telecommunication with the narrow band, high-quality motion images do not deteriorate during transmission and exactly the same quality of pictures can be seen at the receiving end of the network. Thanks to this fact, doctors can recognize precise anatomy to make an accurate diagnosis and can provide good educational materials for remote medical students. We are proud to say that we have accomplished more than we expected in the course of these last couple of years. Here we summarize our achievements both as a record for our own benefit and as a reference for interested friends. This year commemorates the 40 th anniversary of the Korea-Japan friendship treaty. We believe that our project will go beyond the borders of the two countries and will be ultimately beneficial to patients throughout the Asia-Pacific region. 9 Why is INTERNATIONAL exchange important? Big difference in medical standard among countries Medical science spreads easily by reading papers Medical skills have to watch for real understanding We should see what people are doing in other countries. But, only limited number of people can visit other institutions, and just once in a while. There are many things we have to learn by watching in medicine, not by just reading papers. Advantages of our telemedicine project For doctors and institutions - Learn new and different procedures by real watch - Many people at once, and at anytime - Reduce accidents due to immaturity of their skills - No time-consuming trip abroad For patients - Provide better and safer medical care For global health care - Standardization and globalization Kyushu Univ., Fukuoka There are many advantages of our telemedical project. -7-

10 Why Asia? 1. Huge population 2. Medical standards are so different due to diversity of cultures, races, and religions. 3. There are specific diseases in Asia, such as gastric cancer, viruses, SARS, etc. 4. Little time difference and short distance among countries, which makes our telemedicine and face- to-face meeting easier. Asia-Pacific regions are our main target. 12 3. Network structure The network at Kyushu University Hospital is part of the Kyushu University Campus Network. Communicating with external networks is carried out through the SINET (Science Information Network) of the MEXT (Ministry of Education, Culture, Sports, Science and Technology) for most of the traffic of the Kyushu University network, but communication for the research network is implemented through the network for the research and development, QGPOP (Kyushu GigaPOP Project). Thus, communication with the research institutes in Korea is carried out through QGPOP. The bandwidth of the backbone-network in Kyushu University is 1 Gbps but the end is 100 Mbps. Kyushu University and QGPOP are connected at a speed of 1 Gbps. Therefore the maximum bandwidth to and from each segment at Kyushu University Hospital to QGPOP's trunk line is 100 Mbps. QGPOP is the network for research and development. QGPOP has dedicated IPv4 (Internet Protocol version 4) address space ( /16) and dedicated AS (autonomous system), the number of which is The external network comprises SINET, WIDE (Widely Integrated Distributed Environment), APAN-JP (Asia-Pacific Advanced Network- Japan) and KOREN (Korea Advanced Research Network) and is interconnected by the Border Gateway protocol. QGPOP is connected, at 1 Gbps, to almost all networks other than IIJ which is a commercial based network. QGPOP's network is shown in Figure 3-1. As for QGPOP, because of its multi-home configuration using a Border Gateway protocol, the connection with external networks can be determined according to QGPOP's own policy, but the route paths are usually determined by the AS path length only. The joint part of QGPOP and KOREN is specifically called Genkai network. As shown in Figure 3-2, the optical fiber which connects Fukuoka and Busan without relaying, called KJCN (Korea-Japan Cable Network) is used. The Genkai network is one part of the APII (Asia-Pacific Information Infrastructure) which connects Tokyo and Seoul at higher speed. It has been in service since 2003 and still continues to operate today. The Genkai network has made possible high-speed communication at 1 Gbps between the main research institutes in Japan and Korea. -10-

14 4. Instruments / Technical tips Here, we explain how to start a teleconference using DVTS environment. (1) Basic & simple configuration A teleconference can be started with very simple system configuration. Required items are shown in Table 4-1. Table 4-2 Technical tips Item Descriptions Digital Video Cam- - A built-in microphone is too sensitive for teleconference coder use because it causes acoustic feedback (howling). That is why it is not recommendable to use a built-in microphone. - External microphones should be used. Microphone - It should have uni-directionality and low sensitivity to avoid acoustic feedback. Personal Computer - Global IP address must be assigned for each PC. - It should have advanced graphics ability, because the DVTS process demands large graphics resource. - It is not recommendable to use built-in graphics chip sets such as i815, i845g etc. - If there are no PCs that have sufficient processing power, two PCs may be used: one is for sending and the other for receiving separately. Network - DVTS software consumes about 35Mbits/sec bandwidth. - Traffic in excess of 35Mbits/sec must be transmitted throughout from a sending site to a receiving site. Any bottleneck on the way to the remote site prevents effective functioning of the DVTS software. - Transmission of such heavy traffic to the Internet requires network system configuration adjustment and international negotiation by network administrators. -14-

18 (2) Advanced configuration To make the teleconference more comfortable, an advanced configuration should be built. In addition to the basic configuration, the items shown as Table 4-3 below are required. 19 Technical tips relating to each of the above are shown in Table 4-4. Table 4-4 Technical tips Item Descriptions D/A converter - Capable of converting DV signal into S-Video or Video, Scan converter S-Video or Video into DV, and also, S-Video into Video, Video into S-Video. This device is convenient for signal conversion. - The scan converter, like the D/A converter, can convert RGB signal into S-Video and Video signal and is convenient for presentation using a PC. Audio mixer - It should have multi-output buses to avoid acoustic feedback. - Although the audio mixer can mix all audio sources, each output channel must be composed independently so that it will not cause acoustic loop. - For example, the output channel for a remote site should be composed of audio sources only except from the remote site itself. Video mixer - The video mixer device can handle sending various images and is therefore very effective and appealing to the audience. - For example, transition of two images, picture-in-picture image, multi-divided image etc. Echo canceller - This will be used to ensure unfailing cancellation of Feedback destroyer acoustic feedback - It is not, however, so effective when using at a local site alone. - To make sure of cancellation of acoustic feedback, it should be used at both ends (local site and remote site) simultaneously. Projector & screen - This is used for displaying sent images. - Provides both the presenter and the audience with images of themselves during the conference. VPN Gateway - This is used to keep all traffics secure over the Internet. - When performing live surgery or other confidential demonstration, this device is mandatory. -17-

Beyond this diversity, a single virus genome lies behind. Five distinctive speakers, who are seriously committed to investigate these disorders, are invited across the Ocean to Fukuoka city, to join the Asia-Pacific session. From Korea, we also will have an Internet presentation, to find out new way of scientific communication between Asian countries. Therefore, let s sit together, and share our knowledge and experience in EBV and its related malignancies. This is the question, which could be answered only in Asia Pacific Region. We have a new trial of Internet presentation by Prof. Kim, Woo Ho (Seoul National University) with high quality microscope image from National Cancer Center (NCC) in Goyang, Korea in the session. 115 11. Closing Brothers! Let s Unite to Spread Benefits across Borders by Broadband! We look forward to the era of the ubiquitous society with the next generation Internet communication technology. In the fields of healthcare, medicine and welfare, we will be able to manage daily life for the elderly and those suffering from dementia, to effect life-style management, to provide education through e-learning concerning life-style related diseases such as diabetes mellitus and to provide a remote medical consultation service for those patients at home. These should provide the environment necessary for realization of a user-oriented, highly efficient, low cost and precise healthcare system. Our project, which comprises the Asian medical network, is based on a concept similar to that described above. Even from your seat in your own office, you will be able to see and learn even detailed finger movements involved in the newest medical techniques in the world; you will be able to understand in concrete terms conditions of an emergent infectious disease which you have never seen and you will be able to prepare for the approaching spread of the disease; you will also be able to make clear presentations at international academic meetings from a remote location 5,000 km distant from the venue 30 minutes after your medical consultation with your outpatients. Thus, we are aiming to provide the essential environment to acquire the best and newest healthcare techniques without placing additional stress on medical staff (cost, time, anxiety regarding the risk of infection, etc). In rivalry with United States, the European Union (EU) is developing rapidly. Accordingly, many of the world s industries and dense information networks are becoming bipolarized. On the other hand, unification of Asia is slow due to such factors as political instability, economic disparities and religious differences, although Asia contains some of the most populated and/or rapidly growing countries in the world. The delay in development of information services will prove a major factor in the widening of the gap between Asia and US/EU, will contribute to difficulties in making good use of resources and will exacerbate the brain drain phenomenon. In the field of medicine, the slow development of information sharing of advanced medical techniques (e.g. endoscopic surgery, robotic surgery and measures against emergent infectious disease or acts of bio-terrorism), medical ethical consensus (e.g. transplantation, regeneration therapy and genomic therapy) and medical peripherals (e.g. medical devices and medicines) will also be a major contributing factor to disadvantages for -113-

116 medical practitioners, patients and citizens alike. In other words, the promotion of unification of the Asian medical society may contribute to world-wide medical development through the creation of a third medical pole.
