53 從 1999 年 Stephen G Bown 教授的來訪 談光動力治療在台灣至今的發展 陳進庭台大醫學院光電生物醫學中心副教授 ( ) 光動力治療 (Photodynamic Therapy) 是目前發展已趨成熟的一種癌症診治方法 其發展在文獻上的記錄可以追溯至 1903 年由德國教授 Tappeiner 和皮膚科醫師 Jesionek, 探討 Eosin 這一種光敏感性染料其對皮膚癌治療效果的研究 然而臨床的正式應用, 則是直到 1995 年由加拿大 QLT 公司自血紫質衍生物進一步純化出的 Porfimer sodium 在完成臨床試驗後, 才在加拿大 美國 歐洲及日本各國陸續的許可之下成為第一個核可的光動力治療劑 目前在許多國家已被核准作為臨床上使用的光感藥物除了第一代的 Porfimer sodium 外, 幾個藥效更佳 副作用更低的第二代光感藥物亦已上市, 如用於腫瘤治療的 Temoporfin Aminolevulinic acid 和 Methyl Aminolevulinate, 以及用於黃斑退化症 (macular degeneration) 治療的 Verteporfin 1999 年台大醫學院雷射醫學研究中心 ( 自 2000 年起更名為光電生物醫學研究中心 ) 舉辦 光電生物科技暨雷射醫學國際研討會, 這一個會議在雷射醫學的討論主題, 主要即是以 光動力治療 為主 為此, 我們特地邀請來自英國倫敦大學國家雷射醫學中心主任 Dr. Stephen G Bown 來台, 以 Recent Advances in Photodynamic Therapy 為題目, 擔任大會主要講者 隨後在國家衛生研究院的安排下,Dr. Bown 又到台大醫學院及花蓮慈濟大學進行另外兩場專題演講, 講題為 Recent Advances in Laser Therapy 光動力治療在台灣的發展, 也在 Dr. Bown 來訪之後開始逐步展開 由台大醫學院 台大醫院和台北醫學大學組成的跨領域研究團隊, 首先設計出 5- 氨基酮戊酸的藥物輸送劑型, 隨後並利用台大醫院和工研院所合作設計的 LED 光源模組, 在 2002 年開始進行台灣第一起 口腔癌前病變及口腔癌之光動力治療 的學術臨床試驗 隨後, 台大醫院亦派出耳鼻喉部的婁培人醫師到 Dr. Bown 的實驗室進行一年光動力治療的臨床訓練 2004 年 Dr. Bown 更兩度應邀來台參加 台灣外科醫學會第 7 屆年會 及由台灣所主辦的 The second Asian and Pacific Rim Symposium on Biophotonics 國際會議, 擔任會議的 preliminary speaker, 進行目前光動力最新治療的相關演講 在台灣, 由於國人嚼食檳榔的習慣, 口腔癌發生率逐年增高 根據衛生署所公佈的資料顯示, 口腔癌早期的診斷與治療對於此疾病的治癒率是相當重要的 從在台大醫院進行的臨床試驗結果顯示, 相較於傳統手術切除伴隨着手術後整型重建, 光動力治療可以在當日進行且病人無住院需求 而在治療的過程中病人不但能保持正常的生活作息, 其臉部組織的功能也不會受到太大程度的影響 由於治療過後病人臉部組織的功能及結構外觀並沒受到明顯的影響且顯示著極佳的癒後效果, 因此治療後亦無需經由整型手術來重建其臉部外觀 這一結果顯示 5- 氨基酮戊酸光動力治療技術的開發, 將有極大潛力應用於口腔癌病變或其他淺層性癌症如子宮頸癌的治療 未來也可配合內視鏡的使用, 推廣應用於結腸癌 消化道癌 膀胱癌與肺癌的早期診治 二十一世紀醫學研究的重要方向之一是將科技與醫學密切結合的跨領域研究, 而運用光電科技從事生物醫學的基礎研究或臨床診斷與治療技術的發展更已成為此趨勢的主流 以往由於缺乏整合, 因此光電科技領域與醫學界的的研究人才, 不但缺乏共 317

54 同的語言, 對於光電科技領域中可能得以應用於基礎或臨床醫學研究的項目也不熟悉 而光動力醫學的發展即適時提供了一個溝通與合作的技術平台 事實上, 過去 5 年來我們在光動力醫學的發展即整合光電與生物醫學科技, 從基礎醫學研究 藥物劑型設計及新型光感藥物的開發 螢光影像分析系統的建立 用於激發光感藥物的 LED 及雷射光源發展, 到臨床試驗的進行 這一路走來雖然步履蹣跚, 但確也實際地治癒了一些病人使其免於開刀之苦及疾病的威脅, 而在跨領域的合作研究上更累積了許多的寶貴經驗 陳進庭副教授擔任 88 年 4 月 19 日 Series 籌備召集人 318
