1 Basic: Personal Information week 1 Unit objective: s will be able to identify and respond to personal information vocabulary/ questions orally and in written form. Vocabulary: first name, last name, address, city, state, zip code, country, telephone number, social security number, Minneapolis, Minnesota, what s? Monday Lesson Objectives: Materials Name and identify upper and lower case letters Recognize that all letters have names and sounds Ask and respond to requests for first and last name Recognize key sight words (name, first, last, is, my, your, his, her, What s) and use them appropriately Story: What s Next? Introductory, Introducing SamSam and Adam, pg 4-5 Letter flashcards (T-made, ask coordinator) Name mingle (see below) Survival English 1, Activity Ideas: Alphabet match: s match capital and lowercase letter flashcards in pairs. Fast finishers can write the pairs in their notebooks and read them aloud to their partner. Name Mingle: After introducing the sight words, model the questions and answers needed for the mingle. Model with co-teacher/volunteer. Challenge the higher level students to use complete sentences when answering. Optional extension: Have students write his/her sentences afterwards using their answers (Her last name is Nur. His first name is Abdi.) Her/his Worksheet: Survival English 16-17

Ask and respond to requests for address, city, state and zip code Fill out forms with first and last name, address, city, state and zip code Name and identify numbers 1-10 Listening script / worksheet (see below) Mini white boards (see coordinator) Activity Ideas: Chain Conversation s stand in a circle and one at a time ask the person to their left: What s your first name? That person answers: My first name is. Switch directions. Repeat with: What s your last name? My last name is. Can repeat with new language introduced today pertaining to address. White Board Numbers s have mini white boards; teacher calls out a number. Ss write the number numerically. Higher level students write both the number and the word for the number. Repeat for numbers Repeating having a student be the caller. Listening Fill-in Ss listen to the conversations one at a time; T reads. Ss fill in the cloze worksheet while listening to each conversation again.

B: My address is Practice as a class, T/Ss, half class/half class, pairs, pairs perform. Dictation: name, address, first, last, city, state. What is your zip code?
