1 1. GENERAL REQUIREMENTS Refer to HPUK General Requirements for lining stamp, swing tickets, shoe boxes, restricted substances, packing lists, WWW carton labels and carton requirements. 2. UPC/SKU/PRICE LABELS 2.1 SHOE LABELS supplied by HPUK. (This is a 3 part label consisting of both shoe and shoe box labels). An example of the shoe labels are shown below in figure 1. There are 2 shoe labels to be attached centrally to the sole in the waist of each shoe The label without the price is to be placed on the left shoe see figure 1 The label with the price is to be placed on the right shoe see figure 1 Where the sole material is not suitable for adhesive labels these labels are to be attached neatly to the quarter lining. 3 2.3 DOLCIS SHOE CARE LABEL All shoe boxes for Dolcis must contain a Shoe Care Guide as shown in figure 3 This must be placed inside the shoe box with the shoes Hush Puppies will provide this label Figure 3 Written By: Helen Lincoln Date: 8 October 2007 Page 3 of 7

CARTON LABELS 3.1 DOLCIS CARTON LABEL printed by factory The vendor will be required to print outer carton labels as shown in figure 4 (bulk) and figure 5 (pre-pack) The carton label should be of white background printed in black text. Size of label should be 21cm length x 15cm height Carton labels should be affixed securely to BOTH ends of the carton To complete the carton labels use the following information - NAME use Name and colour from the Dolcis shoe label i.e. CLOUD BLACK - LINE NO use 7 digit line no from the Dolcis shoe label i.e L ORDER NO this is the Dolcis order no and will be supplied along with the labels from HP ie SIZE and NO PAIRS - Qty by size and no of pairs to reflect carton contents as specified by the packing list. - TOTAL CTNS IN CONSIGNMENT refer to packing list from GIS or HPUK - SUPPLIER CODE this is always HUS001 - PRE-PACK CODE this will be the last letter of the line no i.e. L071191A will be pre-pack code A this information will be supplied with the labels Written By: Helen Lincoln Date: 8 October 2007 Page 4 of 7

7 3.2 WWW CARTON LABELS provided by Avery Dennison In addition there is a WWW carton label to be attached as well Refer to HPUK General Requirements VCP for instructions on WWW carton labels 4. DOLCIS INTAKE DOCUMENT The Dolcis intake document is to be completed by the vendor for all Dolcis Purchase Orders. See FORMS Dolcis Intake Document Dolcis Order no. and Line no. will be provided by HPUK on a spreadsheet Name is to include material description and colour ie SARDINIA BLACK LEATHER ORIGIN to state factories country of origin for production of the shoes A.P/PACKS and LOOSE quantities per size can be found on the Purchase order All other parts are to be left blank 5. PACKING LISTS Refer to HPUK General Requirements VCP for instructions on packing cartons 6.
