5 We ve had many requests from families who would like to be a part of the EES Birthday Book Program, but who may have missed out on the form at the beginning of the year. So, here it is again! Honor your child s birthday by giving a book to the Essex Elementary School Library! In recognition of your child s special day, you and your child choose a book to donate to the library, and we will place a permanent bookplate with your child s name and birthday and the donor s name inside the book. The bookplates have been specially designed by Bloom Art School owner and children s book illustrator, Linda Bronson. Your child will be the first to check out the new book from the library. All of the Birthday Books will be purchased through the EES PTO. To participate in the program, families may select either a hardcover book for $12 or a paperback book for $5. The PTO is working with Mrs. Giaconia in the Media Center to provide a list of books for this program that will expand our library collection and minimize duplicate books. Approximately 2-4 weeks before your child s birthday, someone from the Birthday Book committee will contact you with a list of books that are available. Please complete all information on the form below and return it with your payment to the EES office. For more information, please contact Birthday Book Coordinator, Jen Pitman, at or jandbpitman@comcast.net. Thank you! EES Birthday Book Program Sponsored by the Essex Elementary School PTO Yes, I want to participate in the EES Birthday Book Program!
