3 Progress Wed July 20 - Wed July 27 Weeks: 11 down, 16 to go Highlights: Magnet rework, measurement and installation going well on the East and West arcs! South side reinstallation complete with alignment hookupcheckout ongoing. C100 tests continue, R100 aligned in final resting spot in zone SL23 RF into shorts for all stations in zone SL24 Progress on in rush currentbreaker issue being made. Installation and testing proceeding elsewhere. All hands safety meetings conducted Areas of attention: Continue vigilance on safety Let s tighten up our work practices and planning Continue vigilance on cryomodule schedule: acceptance tests, installation, test with beam Hall C beam dump work scope/planning etc.

4 CCB The following 6MSD CRs & NTFs have been submitted, reviewed, and approved: Installation of R100 and Admiral & Updated C100 installation plan Decision on Location and installation/commissioning activities (CR) C100 1, C100 2 install as planned in SL24, SL25; Admiral to be removed; R100 installation for SL23 (aligned only). The 6MSD Daily Calendar will be updated the 6MSD progress schedule will show the changes in the 29 July 2011 progress schedule ( Change Requests/Notes to File in the Pipeline: Hall C Current Cavities (CR) Hall C Current Cavities upgrade/enhancement work. CCB reviewed the page one and are working with support resource groups for schedule impacts. Change Requests/Notes to File Upcoming/Placeholders: Tunnel Penetrations Repair (CR) Captures additional detailed scope for the repair work on defined penetrations. Once repair work scope is defined by A&E (end of July 2011), this CR will be reviewed FML Stair Repair at the FEL Block removal for egress (CR/NTF) FML's current plan in the schedule is to repair the FEL stairs. Additional work to remove some of the blocks at the truck ramp roll up door to provide an egress during the repair work. Block removal and crane rental is done by MSINST group. System Testing/Hot Checkout/Restart Operations Plan (CR/NTF) M. Spata is leading the preparation and planning for overall testing, hot checkout and restoration of operations for the 6 GeV run. The plan will be presented and discussed in the second half of August. Hall C Ion Chamber/Dump Work (CR) Hall C ion chamber and dump work being reviewed by engineering division and accelerator operations for scope. More details to come. Plan Still Pending

8 PM&IP Schedule Issues: Hall D Phase 3 Tunnel Extension is ~5 Days behind schedule. Cryomodules C100-1 & 2 have been added to the production schedule. R100 schedule has been descoped. Resource issues: None

28 July 2011 Events Date Location Event 07/01/11 Hall D Construction Site Subcontractor was struck on hard hat with a 2 foot hollow metal spackling rod. Subcontractor was evaluated by JLab Occupational Health Nurse and no treatment was rendered. Subcontractor returned to work with no restrictions. 07/06/11 North Linac Employee bumped his right shin against a blue metal brace causing an abrasion of right shin. Occupational Medicine cleaned the wound, applied over the counter antibiotic ointment, and covered it with a Band Aid. Employee returned to work with no restrictions. 07/07/11 South Linac Coworker spilled PVC glue which then splashed onto right side of student's face and right forearm. Student suffered a first degree chemical burn of right forearm which was evaluated by Dr. Chandler. The exposed areas were irrigated and washed and the student returned to work with no restrictions. 07/07/11 Hall D Construction Site The mechanical fasteners connecting the strut to the linear rail of atrench box in the NE Stub excavation failed. All the other fasteners are in place. No personnel were injured and work inside the trench box was secured by the subcontractor. Access to the trench box has been cordoned off with danger tape. 07/11/11 Hall D Ramp Employee punctured their arm on exposed rebar wire ties in the ramp. Employee was evaluated by Occupational Medicine and was treated for a puncture wound to right arm. The wound was cleaned, and then over the counter antibiotic ointment and band aid was applied. Employee returned to work with no restrictions.

29 July 2011 Events con t Date Location Event 07/11/11 North Access Building Subcontractor slipped in a puddle of water injuring right knee. Subcontractor suffered a right knee sprain and was treated with a cold pack and they were sent off site for further evaluation. Prescription medicine was not medically indicated. Subcontractor is to return to work tomorrow on regular duty. 07/13/11 Hall D Site Subcontractor had sudden onset of low back pain while squatting and was diagnosed with unspecified back injury. Subcontractor was evaluated and treated at emergency department. The subcontractor s work restrictions are that the subcontractor is out of work until 7/16/2011 with, no heavy lifting until cleared by physician. 07/19/11 FEL Vault Student called to report that while brushing off a piece of aluminum, he cut the index finger of his left hand. Student reported superficial laceration to the left index finger and stated that he cleaned the laceration, applied over the counter antibiotic ointment and a bandaid. Student returned to work regular duty. 07/20/11 West Side TED Building During pre shift stretching activity, subcontractor experienced episode of lower back discomfort. Occupational Medicine evaluated and diagnosed subcontractor with lower back strain. Subcontractor took one dose of over the counter medicine prior to coming to JLab Occupational Medicine. Subcontractor returned to regular duty on 7/21/11.

30 July 2011 Events con t Date Location Event 07/22/11 TLA building second floor After working several hours in the TLA building subcontractor began to experience symptoms of heat exhaustion. Subcontractor was evaluated and treated in JLab Occupational Medicine. They applied cold packs and gave him oral fluids. Subcontractor was sent home for remainder of the day and will return to work on next scheduled work day on regular duty.
